NOTICE
WINTER SPORTS REGISTRATION

All students are required to have a current physical examination on file in the Nurse's Office prior to participating in the athletic program in order to keep your child safe and protect confidentiality.   This means that before your child can tryout, practice or participate on a team they must have a copy of their physical examination on file as required by the MIAA guidelines.  A physical exam covers the student for 13 months from the exam date.

If your child's physical exam expires during the sport season, your child will no longer be able to participate until a new physical examination has been obtained and is on file in the Nurse's Office.

If you currently have a physical examination on file in the Nurse's Office and it is NOT due to expire you do not need to provide another copy.

Winter sport physicals are due in the Nurse's Office at WHS by Monday, November 14, 2016.   PLEASE NOTE: Late submission of physicals delays the student from any athletic activity until all the registration requirements are completed.

Please also note the nursing staff will NOT be available on the first day of practice, Monday November 28, 2016.

The Athletic Office or individual coaches will not accept physical examination reports.

Physical examination information should be sent to:
