What is SIM? Experts tell story

Watch the first in a series of videos that outlines the mission for the Colorado State Innovation Model (SIM), some of its goals and a few of its success stories. This short video features many of the faces and voices of people who have been integral in creating, supporting and furthering SIM's mission to integrate behavioral health and primary care. They talk about why it matters to healthcare providers, patients and insurance companies that are working with SIM to create sustainable pathways to integration. Tell us what you think of the video and why access to integrated care is important to you.  

Participate in practice transformation


The Transforming Clinical Practices Initiative (TCPi), a federal program offered statewide for up to 2,000 providers and staff, offers a free support network designed to help practices deliver patient-centered care and navigate compensation changes. Enroll in a short, informational webinar Wednesday, Oct. 12 at 12:30 p.m.

Integrating care in Colorado: A CHSM discussion

As you may know know, Spark Policy Institute (Spark) is working in partnership with the Colorado State Innovation Model (SIM) Office to develop a statewide telehealth strategy to be implemented in 2017. We invite you to be a part of shaping the vision of telehealth in Colorado.
 
Join us on Tuesday, Nov. 22 from 9:00 a.m. to 12:00 p.m. at the Colorado Department of Public Health and Environment for an in-person interactive discussion facilitated by Spark about what the future of telehealth in Colorado could look like and how we can get there.

Funding opportunity from The Colorado Health Foundation: 
Strengthening primary care

The Colorado Health Foundation is pleased to announce a unique funding opportunity for primary care providers that is designed to increase the number of Coloradans receiving quality primary care services through both grant and loan funds.

With the passage of the Patient Protection and Affordable Care Act (ACA), Medicaid expansion and the growing momentum towards outcomes-based payment models, Colorado primary care providers are adapting to a changing environment. The Foundation recognizes that providers are changing their business models and may lack flexible capital for facilities and growth, integration of services and improved data systems. 

The Strengthening Primary Care funding opportunity provides grants up to $500,000 and/or low-interest loans from $500,000 to $5 million for physical and information technology infrastructure as well as working capital related to expansion or improvements to care delivery.
