All In partners include the Community Health Peer Learning (CHP) Program, Data Across Sectors for Health (DASH), the BUILD Health Challenge, and the Colorado Health Foundation - Connecting Communities and Care.

Coordinated Whole-Person Care that Addresses Social Determinants of Health

A new issue brief shares how five DASH-funded projects are developing data infrastructure to support models of coordinated care across medical and community services, orienting care around the whole person's needs, not just their healthcare.  


Community Bright Spots Highlight Insights in Data Sharing for Population Health

The CHP Program, a partnership of AcademyHealth and the Office of the National Coordinator for Health Information Technology, released a series of bright spot profiles highlighting insights from three communities each working toward population health improvements through the expanded capture, sharing, and use of electronic health data from diverse sectors.
 

Building Care Coordination Data Systems to Better Meet Patients' Needs

Integrating data from sectors such as medical, behavioral, and social services enables care teams to proactively identify and address the needs of patients. The latest webinar in the All In project showcase series featured presentations from projects in two communities developing technology solutions to improve care coordination for special needs populations. 


Master Person Indexes: A Tool for Population Health Management

Master Person Indexes (MPIs) help maintain consistent, accurate person data across various health care and community organizations. All In and and the California Health Care Safety Net Institute hosted a webinar featuring experts from two communities, who shared considerations for developing and using MPIs to improve population health.

Introducing: 19 New BUILD Communities!

The BUILD Health Challenge welcomes 19 new communities that will embark on a two-year endeavor to transform health locally. Funder resources totaling $8 million will support multi-sector local partnerships to improve community health. 


Medical-Legal Partnership Helps Align Diverse Partners

A recent Practical Playbook blog shared how BUILD awardee Health Forward/Salud Adelante brought together stakeholders from health care, public health, and the law to connect patients to resources and services that address stressors and ultimately improve health outcomes. 


Project Spotlight

Trenton Health Team

This collaborative, a BUILD 2.0 awardee, initiated a "Safe & Healthy Corridor" along Trenton's North Ward. Aligning with the City's Trenton250 Master Plan, current aims include city revitalization and development of green spaces, and fostering nutrition and active living in local schools and corner stores.

  • Health Impact Assessment GrantsDue September 22  - The Health Impact Project, a collaboration of RWJF and Pew Charitable Trusts, is awarding at least four, two-year grants of up to $100,000 to advance Health Impact Assessments implementation.
  • AHRQ Implementation & Evaluation of Health IT StrategiesDue September 25 - AHRQ is funding projects that stimulate innovative and collaborative research by utilizing new health IT strategies for collecting and using patient-reported outcome measures in primary care and other ambulatory care settings.
  • RWJF Culture of Health Prize: Due November 3 - This prize recognizes communities that have placed a priority on health and are creating powerful partnerships and deep commitments that will enable everyone, especially those facing the greatest barriers to good health, the opportunity to live well.

  • APHA Annual Meeting: November 4 - 8 in Atlanta, GA - During an All In session, four local health departments will discuss how they are accessing, analyzing and leveraging big data to address public health priorities. 
  • Midwest Forum on Hospitals, Health Systems, and Population Health:  Nov. 29 - Dec. 1 in Chicago, IL - This forum, hosted by four Midwestern public health institutes, which will engage attendees in a broad dialogue across sectors to solve complex health issues by integrating clinical, community health and prevention strategies.
