Community Readiness Survey

TSPN, in partnership with the Tennessee Department of Mental Health and Substance Abuse Services (TDMHSAS) encourages your participation in this year's Community Readiness Survey. Details below:

In May 2016, TDMHSAS requested your assistance to evaluate the community's readiness about suicide prevention in Tennessee.

Community readiness is the degree to which a community is willing and prepared to take action on an issue. Assessing the community's readiness on suicide prevention can help the community move forward and be more successful in its efforts to change by identifying our community's weaknesses and strengths and the obstacles we are likely to meet as we move forward.

As such, TDMHSAS is again requesting your assistance by completing a brief confidential survey to evaluate if there has been any change. Please complete the survey even if you participated in May. Your feedback is invaluable. The deadline to complete the survey is October 12, 2016.

Thank you for your participation in this collaborative effort to reduce suicides and attempts in Tennessee.
