REMINDER: 
Tier Request Process Discontinued Effective September 1
 
Please note that, as previously announced, the Division has discontinued the tier request process effective September 1, 2016 and  Request for Tier Assignment Forms are no longer accepted by mail or by secure upload . Please do not attempt to upload tier requests to the secure site, as the "tier request" dropdown option has been eliminated.
 
Mailing of tier assignment letters to individuals who completed the NJ CAT assessment between November 1, 2014 and August 31, 2016 is currently under way. For NJ CAT assessments completed after September 1, 2016, a tier assignment letter is mailed to the individual (and to the guardian, in cases where the individual does not reside with the guardian) 2-4 weeks after the NJ CAT completion date.
