SIM Practices Preparing for Transformation, Integration 

SIM practices preparing for transformation, integration
The SIM team is seven months into practice transformation efforts with SIM cohort 1, a group of 100 enterprising practices and four community mental health centers that were accepted into this initiative, which is funded by the Centers for Medicare & Medicaid Services. The goal is to support the integration of behavioral health and primary care and to test value-based payment models, which will help practices succeed with the Medicare Access & CHIP reauthorization Act of 2015.

We appreciate the work SIM practices are doing to reassess and refine processes that will support a person-centered approach to healthcare and incorporate the social determinants of health. This is difficult work, which is why SIM provides ongoing support and guidance from practice facilitators and clinical healthcare information technology advisors, who help SIM practices chart new paths that lead to better health outcomes for patients and more satisfied healthcare teams. Get Help Selecting the Best Practice Transformation Track

As practices gear up for new payment models and integrate behavioral health and primary care to ensure healthier patients, there are many options for support and coaching. The Colorado State Innovation Model (SIM) is one of several options for primary care practices in the state. The SIM team created a  decision tree survey  to help you identify which program is best-suited to help your practice move along the continuum of value-based care.

Join the Transformation Community: Prepare for the SIM Cohort 2 Application

We will release the application for SIM cohort 2 by the end of November and hope you will help us spread the news about this unique opportunity for medical practices to receive guidance and support as they integrate behavioral health and primary care and prepare for value-based payment models. Approximately 150 practices will be selected for cohort 2 and will receive transformation support beginning in the spring of 2017. 

This valuable package of practice support focuses on healthcare delivery transformation, population health, and health information technology. Get more information about the unique benefits to SIM:


Practices that applied for CPC+ and are interested in SIM can download and save a copy of their PDFs (before Sept. 15) and access a streamlined SIM application this fall to avoid re-entering information asked on both applications. The CPC+ applications must be downloaded by Sept. 15 to benefit from the streamlined SIM application.

The Transforming Clinical Practices Initiative (TCPi), a federal program offered statewide for up to 2,000 providers and staff, is close to capacity. Eligible providers include MDs, DOs, PAs and APNs, LCSWs and psychologists in primary care, specialty care, and behavioral health practices. Providers enrolled in a Medicare shared savings program or the Comprehensive Primary Care Initiative (CPCi) are not eligible.
