To obtain a Wellness Program Incentive (1 for $100 or 2 for $200), you must complete an attestation form by Nov. 30 ^th.

Time for Steps4Staff

Join the Wellness Team at Steps4Staff on Friday Sept. 30. Registration is $15, and all proceeds go to the SAC Staff Educational Enhancement Fund. Staff members can apply to the fund for awards for textbooks, courses off campus, conferences, workshops and other activities that allow them to develop new skills .

What's Coming Up at 121

FREE Guest Week is Sept 5-9. Anyone with a CWRU ID is welcome to try out the Fitness center all week!

The last round of Get Fit for Life starts the week of 9/19. Wellness Participants can use a $40 subsidy for this program!

Time for Fall Activities

In this SECOND newsletter of the month we remind you that Fall is the time for our annual wellness activities:
* Biometric Screening
* Health Risk Appraisal
* Tobacco Attestation Form
Participants are asked to complete all three activities annually. Those Benelect participants who do will receive a $25 incentive in their paycheck each month in 2017.

Biometric Screening

The annual Biometric Screening includes tests for height, weight, bp, blood glucose and more. Participants are required to log onto the Quest website to sign up for a time slot for any of our upcoming onsite screenings. (Registration Key is CWRU)

Those who prefer may also use results from a physician visit since April 1, 2016. Log onto the Quest site to print your personalized screening data form. Quest Patient Service Center visits are also available for those interested.

Complete Your HRA

The Health Risk Appraisal is a free and confidential online survey which can be completed at case.edu/hra . The appraisal asks questions about smoking, drinking, diet, exercise and other factors that influence well-being.

Attest to Your Tobacco Use

In just a few seconds you can complete your tobacco attestation in HCM. Log into hcm.case.edu.

Main Menu - Self-Service - Benefits -Tobacco Attestation Form

Make sure you attest for 2017. You may need to click the "Add Next Benefit Year" button on the bottom of the form.
