Congratulations to the first
ATHN coaches on completing
the eCoach-the-Coach program!

In June 2016, eight coaches received their Certificates of Completion from The Dartmouth Institute Microsystem Academy (TDIMA) eCoach-the-Coach (eCTC) program. The five-month program is an intensive, dynamic and highly interactive action learning series that focuses on the development of knowledge and skills necessary to successfully coach frontline clinical and supporting teams. The coaches attended four electronic learning sessions and a three-day face-to-face learning session. Coaches were paired with a TDIMA-experienced senior coach and a quality improvement team from a hemophilia treatment center (HTC) in a different region and participated in their monthly virtual weekly learning sessions and weekly team QI meetings. These pioneers are the first members of a national network of coaches who will serve as a resource for the regions to increase QI capacity at the HTCs. The NHPCC wants to thank the eight coaches for volunteering to be the pioneers in the quality improvement efforts of the bleeding disorders network.

Voices of the Coaches

"I am not only learning about the different facets of the QI process and how to apply it in clinical microsystems, but I am also gaining other transferable skills, e.g. meeting skills, effective communication skills and learning about myself."

"The program brings team members out of their silos to look at their operations as a whole and work together to improve care. HTCs, by gaining a deeper understanding of their microsystem, appear to target change ideas that they 'own' and are excited to address"

Update of the NHPCC National Quality Improvement Pilot Sites

The transition from pediatric to adult care, a national priority, is the quality improvement focus of the 10 HTCs that volunteered to be pilot sites for the NHPCC Dartmouth quality improvement program. Although all of the centers are addressing the national goal of improving transition, each site has chosen a specific AIM identified through assessment of their HTC microsystem, including available resources, staffing, population served and practice patterns. At the ATHN Data Summit, the HTC QI teams and ATHN coaches will participate in focus groups to discuss the benefits of the program and how the pilot program can be improved to better meet the needs of the bleeding disorder community.

Voices of the Pilot Sites

"It was very helpful for our patients to see our level of engagement
and commitment, which they in turn reciprocated."

"Having the whole HTC team meet to develop our specific AIM
made me realize the importance of team involvement
and communication."

Welcome to Wave Two Coaches and Pilot Sites

In July 2016, 12 coaches began their journey of becoming ATHN improvement coaches through the eCTC program offered through The Dartmouth Institute Microsystem Academy. These coaches will engage in "co-coaching" and be matched with six HTCs to focus on improving the transition from pediatric care. Dartmouth senior coaches will include Randy Messier and Maria Scopelitti. The new coaches and sites will benefit from "lessons learned" from the pilot program and the experience of the current coaches and sites, who will serve as resources during the next year

Action Guide for Improving Care for People
with Bleeding Disorders

The TDIMA and ATHN have customized the materials and tools used in the pilot program for future use. Individuals involved in the Wave One pilot and the NHPCC Quality Improvement Work Group reviewed the guide to recommend changes for further improvement. The Action Guide will serve as a resource for HTCs and regions to begin their quality improvement programs. The HTCs will be able to download and modify the materials that are relevant to their QI program.
Electronic improvement modules, which include videos, examples, templates and other resources, will be available to complement the Action Guide in September 2016, at www.clinicalmicrosystem.org The Action Guide will be available soon.

Correction: An earlier version of this newsletter incorrectly labeled Cincinnati Children's Medical Hemophilia Center in the Western region and the Center for Comprehensive Care and Diagnosis of Inherited Blood Disorders in the Great Lakes Region. This version has been corrected. Cincinnati Children's Medical Hemophilia Center is in the the Great Lakes Region and the Center for Comprehensive Care and Diagnosis of Inherited Blood Disorders is in the Western region. Kara Garner, RN, MSN, NP is coaching with the Hemophilia Center of Western NY and Robert Fox is coaching with the Oregon Health & Science University. They were mislabeled in the earlier version.
