There is an "All Partners" meeting scheduled for September 27 from noon to 4 pm at the Otesaga Resort Hotel in Cooperstown, New York. This is an opportunity to network with partners and hear about the state of the Performing Provider System (PPS). Please be on the lookout for more information on this meeting. Please note the September 27 date is a change from one sent out in a previous newsletter.

Per partner feedback, meetings have been changed from 10 am to 2 pm to 12 pm to 4pm.  We would ask that if you RSVP "yes" to the meeting and are not able to attend, you either send a delegate in your place or notify our office in advance.  We would like to be prudent stewards of DSRIP resources and as we are charged for all ordered meals, it is important that we have accurate headcounts for these events.  Thank you in advance for your participation!
Midpoint Assessment Completed

As part of the submission process for the DSRIP Year 2 first quarter (DY2Q1) report, all PPS's were required to submit project-level narratives, as well as a PPS level executive summary on progress to date and well as any identified implementation risks.  This "Midpoint Assessment" will be evaluated by the Independent Assessor to understand how this work is progressing on a state-wide level.  An additional component of this assessment will include an e-mail survey by the state to PPS providers.  If you are contacted via this survey process and are not sure how to respond, please contact our PPS Lead Agency office at 607-547-7707.
LCHP Recognized for Funds Flow Progress
Although many PPS's have not yet distributed significant funding to PPS partners, LCHP (listed in article as Bassett Medical Center PPS) was recognized by the state for the amount of money distributed to partners to date.  Credit for this accomplishment goes to the high functioning LCHP Finance committee, co-chaired by Jim Vielkind of Little Falls Hospital and Christa Serafin of Sitrin Medical Rehabilitation Center.

 
Palliative Care Pilot

In alignment with DSRIP and Bassett Medical Center Palliative Care Department initiatives, a 6-month pilot program began on March 1, 2016 with a primary goal of formally integrating Palliative Care into the PCMH setting.  
 
A training workshop was held by Bassett, Catskill Area Hospice & Palliative Care and At Home Care on February 27th.  Attendees included five regional APCs selected for the pilot, nine Bassett PCPs who requested training (including two Regional Medical Directors), and six Administrative Managers.  The workshop covered Clinical and Technical content, including Palliative Care eligible patient identification, "Goals of Care" conversations, symptom management, scheduling and documentation.  The five selected APC's will use this training to schedule and see patients over the next three months, with each case being reviewed by the Palliative Care  team and independent auditors.  Best practices will be implemented into future phases which will include volunteering PCP's, followed by all PCPs network-wide.
 
The workshop was considered a great success by trainers and trainees alike.  Dr. Charles Hyman, Chief of Medicine at Bassett Medical Center attributed the success to "a great example of diverse groups within and without Bassett coming together to collaborate on an important issue for the communities we serve."
Performance Measure Update

As we progress into DSRIP Year 2, our PPS has taken an innovative approach to ensuring we meet our Performance Measure goals.  Rather than rely on multiple project teams to keep their eye on these targets, individual leaders or specific teams have been assigned responsibility to develop workplans to achieve performance measure targets.
A specification manual outlining descriptions of the measure, numerator, denominator and diagnosis codes included in each measure has been posted to the DSRIP website to assist partners and leaders in understanding specific components of these measures. 
Performance measure reports have also been updated to reflect full year results from Measurement Year 1 and now reflect our PPS's new performance targets for year 2.   In addition to the original posted reports on measures, two new reports have been created - one shows measures over time periods, and the other shows measure results by "Assigned Medicaid PCP".   A link to the performance measures web page is below:

Little Falls Hospital Piloting Rapid Cycle Improvement

In support of DSRIP Goals, Little Falls Hospital has partnered with performance improvement experts to participate in a "MAX" Series improvement effort focused on ED usage.  "MAX" is short for "Medicaid Accelerated Exchange":  these efforts are intended to use data to identify improvement opportunities and then utilize rapid cycle improvement tools such as PDCA (Plan, Do, Check, Act) to pilot tests of change. 
As part of this work, Little Falls has focused on its Emergency Department "super utilizer" population - those patients who present multiple times in a short time period.  This patient group is analyzed to understand what interventions might help these patients receive appropriate care before they have to resort to ED usage.  So far, this work has resulted in the identification of the need to include community-based navigation resources in the Emergency Department, as well as ensuring patients are established with local primary care providers.
LCHP's next steps in this area include a plan to obtain funding to increase navigator resources, collaborating with other PPS hospitals to begin addressing their own super utilizer populations.  Two other network hospitals are already initiating a MAX-like program into their emergency department population, focused on asthma patients.
 

Lead Agency Awarded Grant for Chronic Disease Management
Bassett Medical Center, lead agency for LCHP, was recently awarded a HRSA grant for chronic disease managemen t. The chronic disease self-management group sessions that will be held for high-risk patients with chronic conditions is designed to improve clinical outcomes. The grant complements the work being accomplished through DSRIP initiatives, including PCMH. Health coaches and diabetic educators will lead this work, corresponding well with the PPS' commitment to address emerging workforce needs in the outpatient setting.

Workforce Update
Workforce Achievement Value worth $3.1M:
The total Workforce Achievement Value (AV) for LCHP is valued at $3.1 M out of $12.6 million slated for organizational projects. The Workforce AV is tied to all 11 clinical projects and is dependent upon completion of the prescribed workforce milestones and required semi-annual data reporting. This includes ongoing partner reporting on staff impact over the life of DSRIP. To ensure LCHP achieves the Workforce AV, Core Teams have been developed.
What are Core Teams?
Core Teams are made up of individuals within the partner organizations who interface with each other and the PPS Project Leads in order to identify, gather, and report the information needed to meet the workforce AV.
Core Teams create a sustainable communication structure to inform and report workforce related activities. Additionally, Core Teams account for turnover and allow partners to engage and own their portion of the work. Core Teams are tied to funds flow and PPS citizenship.

This reporting is due on a semi-annual basis; all three reports must be submitted with each report  to earn one Achievement Value.  There is no partial AV for Workforce reporting.

LCHP Approved to Reorganize as LLC
 
The New York State Department of Health has given its long-awaited approval for our PPS to form a limited liability corporation and assume the name "Leatherstocking Collaborative Health Partners". Though Bassett Medical Center remains the lead agency in this endeavor, it permits our PPS to have its distinct identity in this transformational work.
As many recall, it was our partners in the early phases of the DSRIP work that chose the Leatherstocking name. All correspondence (including letterhead, business cards, etc.) will soon reflect our new name.
