3 Ways Communities Are Sharing Data to Improve Health

As DASH documented data sharing initiatives across the U.S. through an environmental scan, three primary community health aims were revealed: 1) care coordination, 2) community health needs assessment, and 3) research and policy change. This blog shares key insights related to each aim and includes examples of initiatives that are integrating multi-sector data for these purposes.

Sharing Data Across Sectors for Better Complex Care

A new blog from  Camden Coalition highlights how they are working with experts in complex care across the country, including DASH and CHP projects, to build an evidence base for the treatment of complex health and social needs nationwide. Learn more about their role in working with All In to improve community capacity to address the social determinants of health through the use of data.

California Accountable Communities for Health Awards Announced

Congratulations to the six communities that received funding through the California Accountable Communities for Health Initiative (CACHI), including Be There San Diego, a collaborator on the San Diego Health Connect project, a CHP Subject Matter Expert Community! Project Spotlights

This month, we're featuring two projects that are integrating data from multiple sectors to improve care coordination for complex, high-need populations.

DASH: Altair Accountable Care for People with Disabilities

Altair Accountable Care Organization (ACO), with Lutheran Social Service of Minnesota as the lead member, is pioneering a new delivery model that includes "disability-competent" behavioral health providers in a Health Information Exchange. Through their DASH project, Altair ACO is enabling behavioral health providers to measure and track events in the HIE in real time so that the care team can intervene in a timely and appropriate manner.


CHP: Vermont Child Health Improvement Program, University of Vermont 

The Vermont Child Health Improvement Program (VCHIP) of the University of Vermont College of Medicine's Department of Pediatrics is a population-based maternal and child health services research and quality improvement program. Through its participation in CHP, VCHIP is creating an electronic shared plan of care to improve outcomes for families with children and youth with special health care needs by enhancing care coordination and system integration.

  • National Committee on Vital and Health Statistics: Sept. 27 in Washington, DC - Representatives from All In are attending this national meeting to identify gaps and opportunities for using data to promote multi-sector approaches for community health.
  • APHA 2016: Oct. 29 - Nov. 2 in Denver, CO - Come to our session to hear how Public Health Seattle King County (DASH grantee) and San Diego Health Connect (CHP grantee) are driving population health improvement through data sharing and collaboration.
