Braiding Initiatives: Next Steps to Taking Action 

Braiding ACP components with initiatives that affect students with disabilities will provide a catalyst for their successful transition. District teams will use the Transition Improvement Plan to examine their district's post school outcomes & researched based transition opportunities to increase post school outcomes for students with disabilities.
