Aetna Better Health of Ohio is offering a health equity program focused on African-American members who have Hypertension. The goal of this program is to engage the members by helping them become more participative within the health care process, specifically in managing their hypertension.
The program will span for a duration of 6 months to a year. During that time frame, members will be encouraged by their Case Manager to visit their primary care physicians at least 2 times. The expectation for each of the visits is to ensure that the member is successfully engaged in dialogue and education regarding their hypertension. This will include topics on blood pressure monitoring, medication adherence and lifestyle changes for managing their condition.
Furthermore, we would appreciate your support throughout the program as we will be requesting blood pressure results and prescribed medications at both the beginning and completion of the program. These results can either be submitted via faxed documentation or by verbal confirmation from one of your clinical staff.
