2017 McDonald Criteria for MS
The February 2018 issue of The Lancet included an updated position paper from the International Panel on Diagnosis of Multiple Sclerosis . Three major takeaways for the laboratory include:

CSF oligoclonal bands (OCB) are now considered diagnostic of MS after patient's first attack if more than two MRI lesions are present.
CSF OCB are diagnostic after a second clinical attack implicating a different CNS site, even if only one MRI lesion is present.
More than two CSF OCB indicate intrathecal antibody synthesis more reliably than other CSF tests.

The SPIFE Touch CSF IgG IEF is a "gold standard" method of IEF followed by Western Blot. It has a 95% sensitivity and specificity, far superior to standard electrophoresis or IgG index methods, with a dynamic range of 0.025 mg/dL to 5.0 mg/dL.
