Florida Clostridium difficile Infection (CDI) Cohort Monthly Update
January 2018 CDI Data is Due by February 10.
Be sure to review the "Alerts" tab in the NHSN and complete missing and/or incomplete data for previous months in order for your facility to receive credit for data submission.
Upcoming Deadlines Approaching!
DUE January 31, 2018 : 2017 NHSN Annual Facility Survey for Long-Term Care Facilities (LTCFs)

If you haven't already done so, please submit your 2017 NHSN Annual Facility Survey in the NHSN web application.
  • It is recommended that users complete the paper version of the survey (for data collection purposes only) prior to entering the data in the NHSN web application.

  • We recommend that you print a copy of your previously completed 2016 NHSN Annual Facility Survey for reference when completing the new survey.
  • To print your previous survey for reference, select 'Survey' then 'Find' and Survey Year = 2016.
  • Please note: The 2017 NHSN Annual Facility Survey must be completed in its entirety before the NHSN web application will save the information.
  • After completing your 2017 Annual Facility Survey, please add monthly reporting plans for January 2018 through December 2018.
DUE April 30, 2018 : Mandatory NHSN Re-Consent and Agreement to Participate

CDC Cohort members must complete an electronic Re-consent and Agreement to Participate form to replace the previously submitted paper version uploaded at the beginning of this project. The Re-consent and Agreement to Participate is now available via your Alerts tab in NHSN and takes no more than one minute to complete.
  • For current NHSN facilities, either the NHSN primary contact for the facility or the assigned NHSN facility administrator will need to log into NHSN and accept the new Agreement to Participate and Consent (or re-consent).
  • HSAG is kindly asking CDI Cohort Members to accept the new Agreement to Participate and Consent by April 30, 2018.
  • If a nursing home fails to re-consent, its NHSN functionality will be suspended until the consent form is accepted.
Monthly Reporting Plans (MRPs) for January 2018 –December 2018 
The MRP informs CDC that the facility is participating in the CDI Module. An MRP must be completed prior to being able to submit data each month.
To enter an MRP for each month:
  • Select "Reporting Plan" > "Add" > Month > Year
LabID Event Module Only:
  • Under "Specific Organism Type", select "CDIF-C. difficile" and make sure the "Lab ID Event All Specimens" box is checked, then select "Save".

Why do I Need an Additional NHSN User?
  • Health Services Advisory Group (HSAG) and the Centers for Disease Control and Prevention (CDC) highly recommend that each facility add at least one additional active user in the event that the primary user is on vacation or no longer with the facility.
Reassign Role if NHSN Facility Administrator is Leaving the Facility
What to do if the NHSN Facility Administrator is Leaving the Facility
  • HSAG understands that nursing home staff turnover happens for different reasons
These steps must be taken if the NHSN facility administrator is leaving the facility:
  • Reassign the role to another member of your facility prior to the administrator's departure. This will eliminate a delay in data reporting.
  • Only the user listed as the NHSN facility administrator can reassign the role to another user
  • Reassigning NHSN Facility Administrator
  • After logging into NHSN, click on "Facility" on the left of the navigation bar.
  • Click "Facility Info", scroll down to the bottom of the "Edit Facility Information" page to see the contact information section of your NHSN facility.
  • Users with administrative rights can reassign the component primary contact role to another active NHSN user by clicking on the grey "Reassign Button", searching for the selected user, and clicking the grey "Submit" button on the view user screen once the new primary contact has been selected.
  • If the newly assigned NHSN facility administrator does not have a SAMS account, he/she will need to complete the SAMS registration process.

  • If the NHSN facility administrator leaves the facility without reassigning the role,
  • A formal letter is required by the CDC to request the role change
