3rd Annual DSRIP Learning Symposium

The annual DSRIP Learning Symposium was held February 6 th-8 th and hosted on Staten Island. Over 700 state and international guests gathered to collaborate, learn from experts, and showcase successes. Staten Island PPS had the opportunity to present 9 different posters spanning over 5 categories of Transform, Heal, Partner, Innovate, and Learn. Mary Han, Director of Continuing Care and Quality Management, and team won best poster presentation in the Heal  category for their poster, "Optimizing the Quality of Life for Patients with Chronic Illness through Palliative Care". We were also privileged to host 2 panel discussions,  including "Building Cross Sector Partnerships for Diversion, Public Safety and Health" and "Preparing Community-Based Practices for Value Based Medicine."

On February  9th, Staten Island Partnership for Community Wellness hosted a buprenorphine certification training at  Richmond University Medical Center  along with  NYC Department of Health and Mental Hygiene made possible by Staten Island PPS's Behavioral Health Infrastructure Project (BHIP). The goal of the training is to expand the capacity of Staten Island health professionals to offer medication assisted treatment to help Staten Islanders reach recovery from opioid use disorder. 

The training included 34 clinicians from various backgrounds and departments, willing and enthusiastic at the prospect of helping Staten Islanders achieve recovery. You can learn more about the training here.

Childhood obesity rates on Staten Island are high, affecting the healthy development of children and leading to diabetes, asthma, and other chronic illnesses. Our children's abilities to reach their full potential are largely dependent on their health.

The Staten Island Child Wellness Initiative (CWI) believes that all children deserve a healthy start in life. CWI is a collective impact initiative aiming to bring 80% of Staten Island children to a healthy weight by 2026. CWI focuses on healthy eating, physical activity, and community-wide partnerships to bring children and families the resources to live healthy lives.

"Our goal is transformation," said Joseph Conte, SI PPS's executive director. "The ultimate goal is wellness and managing chronic conditions in the right setting."

Staten Island Advance included a write-up detailing the Medicaid transformation taking place across Staten Island through Staten Island PPS and its partner network in their February 11th issue. They mentioned some of our initiatives and programs that we're working on to  dramatically reduce the number of avoidable hospital visits occurring on Staten Island. 

NYS DOH PCMH Incentive Payments Revised

Due to the Medicaid Global Spending Cap on PCMH incentive payments, NYS DOH will be modifying its payment arrangements. Starting May 1, practices recognized under the NCQA 2014 Level 3 or NCQA 2017 standards will receive a temporarily reduced MMC incentive payment of $2.00 PMPM for 2 months and the PCMH FFS incentive add-on amounts will remain unchanged. All incentive payments for practices recognized under NCQA's 2014 Level 2 standards will be permanently eliminated for both MMC and FFS.
 
Beginning July 1, 2018, the PCMH incentive payments will be modified to align with the principles of Value Based Payments (VBP). The Department anticipates that later this year, the incentive will be tied to whether providers have a VBP contract. Projected rates for the MMC PMPM range from $5.00-$6.00 for providers with a VBP contract, and around $2 for those without. Additional guidance and educational materials will be published in the coming months outlining the new PCMH incentive payment amounts effective July 1, 2018.

JCC Opens M emory Center For Individuals Recently Diagnosed with Early Stage Dementia/Alzheimer's Disease
On February 12th, the JCC opened a memory center  for individuals recently diagnosed with EarlyStage Dementia/Alzheimer's Disease called "My Place: An Early Stage Memory Center." "Early stage" refers to individuals who are diagnosed with Alzheimer's disease or related dementia , and are in the beginning stage of the disease. The early-stage social programs offer meaningful activity and socialization  in a friendly and supportive environment, and they allow participants to remain healthy, active, engaged and independent.

The Department of Health launched their DSRIP Communications Corner, a new initiative intended to provide insights to PPS on what is in the news regarding DSRIP across New York State. Additionally, upcoming events across PPS will be included.

Value Based Payment Corner

VBP Resource Center Launch
 
SI PPS is excited to announce the launch of its Value Based Payment Resource Center. These resources will be hosted on the SI PPS website and support our overall initiative to educate and prepare our Partner Network for the transition from "fee-for-service" to "value based payments," including the SI PPS VBP Education Program.  In most cases, siloed providers in New York are paid through payment models known as "fee-for-service." This means that providers working independently are paid based on volume instead of quality. By 2020 a transition from fee-for-service to value based payments (VBP) will occur as mandated by the New York State Department of Health. The transition to VBP will reform today's payment models to improve patient outcomes and lower overall cost.
 
Our VBP Resource Center will:
  • aggregate material presented and distributed through our education sessions
  • include additional information deemed particularly useful
  • serve as a simple one-stop shop for our partners to build a foundational and common understanding of VBP
  • prepare organizations for VBP accordingly
Our DSRIP programs are aimed at improving health outcomes and patient experience. The transition from the current fee-for-service model to value based payments will be the next step that sustains DSRIP initiatives. The first curriculum topic of the SI PPS VBP Education Program introduces why value based care is so important and what the transition to value based care will look like.
