All In: Data for Community Health is a learning collaborative that helps communities build capacity to address the social determinants of health through multi-sector data sharing. All In partners include the Community Health Peer Learning (CHP) Program, Data Across Sectors for Health (DASH), the BUILD Health Challenge, the Public Health National Center for Innovations (PHNCI), and the Colorado Health Foundation - Connecting Communities and Care.

How Hospitals and Health Systems Can Move Upstream to Improve Community Health
A new report from the de Beaumont Foundation, in partnership with the BUILD Health Challenge, examines how hospitals and health systems can drastically improve their community engagement work by fostering cross-collaborative partnerships focused on upstream factors affecting community health outcomes.
Transforming Public Health Systems: 21st Century States
With support from RWJF, three states (Ohio, Oregon and Washington) are participating in a learning community supported by PHNCI that aims to test and implement the systems transformations required to ensure health equity. PHNCI has developed new resources to chronicle their progress, including stories and a summary of legislative activities.
Innovative Approaches to Improve Precision in Public Health
A recent webinar featured two
All In projects, led by health departments in Chicago and Baltimore, that are using methods like predictive analytics and hotspotting to target resources more efficiently for greater impact. Watch the recording and read the key takeaways.

Garrett County Health Department's Digital Platform Addresses Community Health Concerns
For one rural Appalachian community, a
n online planning tool created by the Garrett County Health Department (GCHD) was a radical shift away from the traditional bureaucratic process of creating a community health improvement plan. With funding from PHNCI, GCHD aims to customize its planning tool for use by other communities.

How the Aftermath of a Shooting Changed a Collaborative Transportation Project
Just as FLOURISH St. Louis, a
BUILD Health awardee, was ready to launch their collaborative effort to address transportation issues for low-income mothers, the aftermath of a local tragedy forced them to shift perspective. Advisory Board summarized what they learned.
When Social Needs Are Medical Needs
Mike Fried of the Baltimore
DASH project wrote an article in
Slate about how we can change our high-tech health care systems to better address social factors that have an outsize impact on health outcomes.
New York City Neighborhood Health Atlas
The NYC Department of Health and Mental Hygiene, a former
DASH grantee, released their Neighborhood Health Atlas providing data on about 100 measures related to health and social factors for 188 neighborhoods.

- Population Health Learning Network: Due February 9 - Center for Care Innovations is launching a network that brings together CA safety net primary care organizations to strengthen and advance their population health management strategies.
- Culture of Health Leaders: Due February 15 - This RWJF program will develop a large group of leaders from diverse sectors to work with organizations, communities, health systems, and policymakers to build a Culture of Health in America.
- J-PAL State and Local Innovation Initiative: Due March 2 - This initiative supports U.S. state and local leaders in using randomized evaluations to generate new and widely applicable lessons about which programs work, which work best, and why.
- State and Local Innovation Prize: Due March 6 - Milkbank Memorial Fund and AcademyHealth have teamed up to establish a new annual award that recognizes state and local efforts to advance innovation using data to improve health.
