2017 Staten Island Primary Care Symposium

On Saturday, December 2nd, over 100 healthcare providers and their staff came out to the Regina McGinn Medical Education Center to partake in the 2017 Staten Island Primary Care Symposium, co-hosted by  Staten Island University Hospital,   the Office of the Staten Island Borough President , and  Richmond University Medical Center. We started off the day with a breakfast and a keynote on Social Determinants of Health from Dr. Ram Raju, Senior Vice President and Community Health Investment Officer at Northwell Health, New York. Guests were then able to choose between three breakout panel sessions:  Advance Care PlanningBehavioral Health Integration, and  Health Information Technology. We reconvened after lunch to hear from Dr. Susan Beane, Vice President & Medical Director of Healthfirst who joined us for a session about  Value Based Care. After a brief intermission, guests were able to choose a second breakout panel session between  Diabetes ManagementAvoiding Hospital Use through Care Coordination, and  Pediatric Wellness. At the close of the symposium, we had the pleasure of awarding 8 practices with the  Primary Care Transformation Award, which was awarded to practices with:
  • NCQA 2014 Patient-
    Centered Medical Home (Level 3)
  • Co-Located Behavioral Health Specialist
  • Meaningful Use Certified Electronic Medical Record
It was a great day of learning, professional development, and networking. Attendees got to hear from leaders who are transforming healthcare across the region and from local providers who are driving change in our community.

Staten Island PPS Rolls Out Social Determinant of Health (SDH) Screening Tool and Referral Resources

Staten Island PPS started rolling out our social determinants of health screening tool and referral resources to empower our clinical and community based partners to screen their clients for social and lifestyle needs impacting their health and well-being. The PPS reviewed five existing tools aimed at screening for social determinants of health. A crosswalk of the five tools was developed to evaluate the effectiveness of each question set related to each specific social determinant category. We ultimately combined question sets from Health Leads and a SDH tool developed by Mt. Sinai PPS. 

Two of our partners, Victory Internal Medicine and E.G. Healthcare, have already updated their electronic health records to include the screening tool and will begin collecting data. 
 
We've collected the most frequently referred-to resources for each of the categories from local community health workers, PAM navigators and health coaches to ensure our providers make on-the-spot referrals for their clients experiencing a need at the time of their visit.

Local Hospital Residents Teach Asthma Education at PS 57

Dr. Ginny Mantello, Director of Health and Wellness at the Staten Island Borough President's Office, has been connecting local schools to asthma resources. PS 57 was the first school to roll out asthma education in their after-school program as part of the Staten  Island Asthma Coalition Initiative. Staten Island PPS funded over 30 SIUH and RUMC Residents to become trained in the American Lung Associate's (ALA) Asthma Basics, which was then taught to the children. Asthma Basics is an interactive learning module designed to help those learn more about asthma. 

The program teaches participants to:
  • recognize and manage triggers
  • understand the value of an asthma action plan
  • recognize and respond to a breathing emergency

PS 57's Asthma Awareness Day was held on November 27th to celebrate those who successfully finished the course. At this event, the Asthma Coalition partners ( American Lung Association, Make the Road New York, SI CARES) along with, Staten Island University Hospital and Richmond University Medical Center representatives were present to provide more information on managing asthma and to celebrate the new asthma champions.
NYS Health Foundation's Population Health Summit

On December 11th, members of the PPS team went to  The New York Academy of Medicine to showcase the work that they've done at the Population Health Summit 2017,    sponsored by the New York State Health Foundation and co-hosted by the Department of Population Health, NYU Langone Medical Center.

The summit featured a diverse set of  perspectives from national, state and city leaders who  shared their strategies for tracking progress, their lessons learned, and opportunities to expand and advance population health efforts in New York State. Panel sessions focused on the continuum of strategies to address broader determinants of health, as well as the multiple approaches underway to tackle the opioid epidemic.

College of Staten Island Will Offer MS in Healthcare Management

Aimed to launch in the winter 2018 semester, the College of Staten Island will be offering an MS in Healthcare Management course.  The MS in Health Care Management is a degree which offers a curriculum with an emphasis on Health Care Administration, including courses in Human Resources/Labor Relations. This is a needed course of study considering healthcare is the largest employer on Staten Island. This is a continuum of our collaborative efforts with CSI to:
  • Create degree bearing certificate programs for Community Health Workers and Care Management
  • Provide scholarships in Social Work and Mental Health Counseling
  • Create Registered Apprentice programs to establish career pathways and hiring pipelines
