Greetings!  Having recently celebrated the Thanksgiving holiday and with the faith-based holidays upon us, we at Capstone have much to be thankful for and know that many of you do, too!  One of our company Fundamentals that we strive toward is -- #21 - Embrace Change: "Nothing stays the same. Change creates energy, excitement and learning. Be inspired by the challenges and possibilities that change creates." We approach the close of 2017 hailing numerous changes among our clients, our services and our own team, which has now passed 30 talented and dedicated members. We have changes in mind for 2018 also - including additional supports for clients to improve compliance and revenues, expanded service lines and new software products. If we've had the pleasure of working with you, let me say "thank you" on behalf of our entire team, for allowing us to serve you and for helping us improve and grow. Meanwhile, we wish that this season provides each of you, our readers, satisfaction and joy as you carry out the mission of your organization.

Our service to clients involves reading encounter notes for coding, auditing and training purposes. We read A LOT of notes. Sometimes we see portions of notes that are str ikingly similar to previous documentation. Copy/pasting, also kno wn as cloning, enables users to select information from one source and replicate it in another location. When doctors, nurses, or other clinicians copy/paste information but fail to update it or ensure accuracy, inaccurate information may enter the patient's medical record and inappropriate charges may be billed to patients and third-party health care payers. Furthermore, inappropriate copy-pasting could facilitate attempts to inflate documentation and generate payments that could be considered fraudulent.

Coding Lymphoma

Lymphoma is a type of blood cancer originating from the lymphatic system. Lymphomas are classified into two main categories, Hodgkin's and non-Hodgkin's lymphoma.

Hodgkin's lymphoma affects the lymph nodes or extranodal lymphoid tissues, such as stomach, spleen, lung, bone and skin.  Most often, this disease originates in a lymph node and spreads to adjacent nodes or to extranodal tissue via the lymphatic system. The most common symptoms are often painless, enlarged lymph nodes (neck, armpits or groins) with other general symptoms such as fatigue, night sweats, fever, chills, and persistent fatigue.  Hodgkin's lymphoma is typically seen between the ages of 20 to 40 and beyond 60 years. 


John C. Stolze
New Senior Vice President of Business Success Announced

John C. Stolze has joined Capstone Performance Systems as senior vice president of business success. Stolze brings to Capstone an in-depth understanding of the operations side of PACE, accountable care organizations (ACOs) and physician groups with a strong suit in value-based care.  He most recently served as director of operations and population health for St. Anthony's Physician Organization in St. Louis, MO.
 
STAFF NOTES:
Capstone Welcomes New Staff

Capstone Performance System's client list continues to grow! To accommodate that growth and ensure that our clients continue to receive the same high-quality services, we are pleased to announce the addition of five new staff members. Risk adjustment and coding specialist Shannon Rockwell, CRN-C, joins Capstone from West Virginia University Hospital where she was an operating room nurse. Shannon received both her RN-Coder accreditation and her nursing degree from Community College of Allegheny County. Annette Nagle, RN, BSN, CPC, risk adjustment and coding specialist, previously served as a clinical chart review nurse and certified professional coder for Tandigm and as a high risk nurse case manager for Healthways. She received her BSN from Bloomsburg University.

Jean Lawton, CPC-A, Amy Tedesco, CPC, and Denise Wills, CPC-A, are the newest members of our coding staff. Jean previously worked as a transcription report clerk for Heritage Valley Health System. Amy served as a manager for the Payment Integrity department with Centene Corporation and prior to that she was a coding manager for Essence Healthcare. Denise comes to Capstone from Excela Health Medical Group, where she was a medical biller. 

Fall 2017 Conferences and Meetings

This fall was packed with activities, including the National PACE Association (NPA) Conference in Boston, the American Academy of Home Care Medicine (AAHCM) Annual Meeting in Chicago, and the CalPACE Member Dinner and Affiliate Meet and Greet in Sacramento. Capstone Performance Systems was fortunate to be a sponsor and/or exhibitor at each of these events. We were thankful for the opportunities to get together with so many of our clients and meet many new friends and contacts.

Several Capstone consultants presented well-received educational sessions at the NPA conference.  George "Mike" Brett, MD, chief medical officer, and David Wilner, MD, FACP, AGSF, senior medical consultant, co-presented "50 Tips in 50 Minutes: Operational Tactics to Optimize Clinical Documentation, RAPS Data and Risk Scores" with Lori Pigeon, NP, Harbor Health Elder Service Plan. Matt Zimmerman, director of client success and risk adjustment consultant, and Deborah Quillen, of our sister company CareKinesis, Inc., co-presented "Ten Easy Steps to Part D Compliance and Tales from the Dark Side: Real-Life PACE Plan Audit Experiences" with Amanda Boyle, BSW, of Immanuel Pathways; Bushra Hashmi, MHSA, MBBS, of Midland CareConnection; and Thelma Pittman-Alderson, MBA, of Cherokee Elder Care. Capstone Performance System's Clinical Documentation Excellence (CDE) Bulletin, is a monthly e-bulletin with topics relevant to Primary Care Providers.
