We're pleased to announce that several providers are now offering telemedicine services to our members! To receive payment, participating providers must submit a completed and signed Telemedicine Attestation Form.

Providers who have billed QualChoice for telemedicine services prior to submitting a Telemedicine Attestation Form will receive the form by mail. Those providers will need to complete, sign and return the form to QualChoice no later than March 1, 2018 to receive reimbursement.
