KNEADED NOTES
November 2017
A letter on why do I want continuing education
Lessons from just one 2 hour education session can directly be applied to Practice:

At the National Convention in California I attended the In Safe Hands: Clients with Diabetes facilitated by Julie Goodwin. I went into this class with an open mind, but thinking it would be review of what I already knew about working with my clients with Diabetes. I came away with gems of information that were new to me, and I can implement in my practice immediately. AMTA offers class notes online via the AMTA website. Also, you can google 'Julie Goodwin' and inquire for more information. Here is some  of the invaluable information from the session.

www.txplanner.org is a valuable tool which can help you with treatment planning. Also, do you ask your insulin dependent diabetic clients what their current blood sugar levels are? If they don't know you can ask them to check. If it is low, you can have water and candy available for them. Julie suggested
small  tubes of icing  f ound at the grocery. They are 100% sugar, and offer quicker recovery of low blood sugar. Wait 15 minutes and test again. Know signs of  ketoacidosis,

Another takeaway: frozen shoulder for diabetics is not the same as non-diabetic clients, and there is little that can be done on our level of approach. You might not be doing anything but damage if you work
deep on a diabetic frozen shoulder. Pain with no gain.

Lastly, an important concept I was not aware of is that you must avoid manipulating insulin injection sites. You might be tempted to break up adhesions in the injection area - DON'T! The fibrotic tissue that has formed from insulin injections can be holding unabsorbed insulin that manual manipulation might

liberate, and complicate their blood sugar balance while they are on your table, compromising their   health! Yikes! Class notes online include a body map of these areas.

I suggest you connect to the AMTA website for notes and contact Julie Goodwin to attend a class if you find this informative and want more information.

Your AMTA-IN Board and Volunteers answer the question...

What is something you currently love about being a CMT, that you weren't expecting when you started your career?

"I have received a level of friendship, spiritual, mental and physical support (gotta love trades!) and professional guidance from associates and clients alike. I went into massage expecting to be giving and was surprised at how much I received and grew personally from my work. Equally unexpected is the flexibility private practice allowed me to be the mom, wife and professional I had struggled to find balance with in other occupations."

"The thing that amazes me the most and makes me love my job even more is the wide variety of clients that I see. I work on old and young, athletes and disabled and everyone in between. Everyone, from every walk of life can benefit from massage.  I love continuing education for just that reason.  As I am in class, I can always think of someone that the work I am learning will benefit. My clients really like the new things I learn. So I guess I also didn't expect to like continuing education as much as I do!"

My career as a massage therapist has changed my whole life for the best. I am genuinely happy! Whenever I am working, everyone I see is glad to see me. I'm confident I can help each client feel better and am pleased by the instant, often dramatic improvements they experience. My work atmosphere is relaxing and pleasant. What's not to love?"

"I did not know how much helping other people through touch could be so rewarding.  I became a therapist to help my autistic son and to maybe do a little massage in my chiropractic assistant duties.  I had no idea how much I would love this career.  24 years later I feel like me going to a massage school to get the name of a midwife was all just part of the plan to put me on my path to today.  AMTA and continuing education keeps me excited and fresh.  Connecting with my patients/clients on a personal level and helping them to find pain free days without the use of opioids and other pain meds is priceless."

Recap:  AMTA Indiana Chapter welcomes Dr. Joe Muscolino Fall Education

"In my opinion the most important thing that we learned from Dr Joe Muscolino was proper body mechanics while doing spine and rib cage work.  The only way to have a long career is to to take good care of ourselves. We give so much to our clients, but often forget to take care of ourselves.  All of Dr. Joe's classes are full of amazing information and he always includes the proper way to position ourselves to do the best work."
-Terri Brandt

"Everyone who knows me as a massage therapist knows I'm a complete fangirl when it comes to Dr. Joe Muscolino! I have signed copies of all his books, subscribe to his Digital COMT, and have taken all the in-person classes required for COMT certification. I admire him because his teachings empower the therapist to think critically and then creatively apply our assessment and treatment techniques. He has distilled anatomy, physiology and kinesiology into his own very clearly-written and beautifully illustrated textbooks and posters.

Dr. Muscolino's main lesson is that if you know a muscle's attachment points, you can figure out its motions. If you take the time to really learn muscle anatomy, you will be able to determine the issues your clients need targeted. Then you need to understand and practice good body mechanics in order to apply massage therapy without injuring yourself. Joe focuses on helping his students determine what needs massage and how to apply our trade effectively.

He sprinkles his teachings with gems that each of us value. One of his most profound jewels for me is that palpation and treatment are often identical, just with different intent. A good, thorough palpation can be an excellent treatment.

Joe saved my thumbs, which saved my career. When I took my first COMT class, my thumb joints were hurting pretty badly. Dr. Muscolino is good at correcting his students' body mechanics when we are practicing, and he teaches good body mechanics. He noticed I was performing effleurage with my thumbs extended, putting stress on my metacarpophalangeal joints. He explained to the class that often schools begin teaching effleurage with gentle Swedish massage, and we learn poor body mechanics. By the time we are practicing deep tissue strokes with heavy pressure, we begin injuring ourselves. That is why many massage therapists leave the profession after only a few years due to injury. It took practice, then time for my body mechanics to improve and my thumbs to heal, but they did and I am usually pain-free." -Susan Moody

Little Gems picked up by Kim Pucka:
  1. "Severity is not the enemy, the fight will come from how chronic the problem is.  If you don't change your direction you are likely to end up where you are heading."
  2. How often should you see your client? Ask this question. Do you only work-out once a week?  Do you see results when you exercise sporadically?  More visits in the beginning of care and then taper off.
  3. I have attended the other Muscolino classes and using joint mobilization in my practice is important to prepare the joints for chiropractic care.   Working just pass the tissue barrier give the best result.
  4. Dr. Muscolino teaches to the level of the class. He engages with every member in the class, is witty and challenging in his presentation.
  5. Dr. Muscolino offers a subscriber online course (www.learnmuscles.com)  and travels around the country.  He has done classes in our neighboring states as well.  "The goal of COMT Certification is to empower the therapist to think critically and then creatively apply their assessment and treatment techniques."

Government Relations UPDATE

From Laura Brown and Amy Levander of Krieg DeVault:

The massage therapy rules subcommittee made substantial progress in the language for Continuing Education and Advertising criteria at the most recent  meeting.  As a reminder, these rules are drafted as a starting point for discussion, some form of these rules will eventually be a part of the final rules adopted by the MT Board.

In Continuing Education, the board looked at rules from several other states to see what they had in common and would work for our state.  The draft included: What education would be accepted, approval process for providers, waivers, appeals, carry-over credit language, and the inactive status language. The last three section might eventually be removed because there is already general appeals process for all professions; calculating carry-over credits is administratively difficult for the PLA; and the MT Board does not have the authority to determine whether a license is inactive or not (licenses are either active or expired).

In the Advertising draft, it will be required to display the massage license in a conspicuous place.  AMTA National Representative requested that the draft be updated to permit massage therapists to either display their licenses, or as an alternative, display a sign that says all licenses are available for inspection (and would need to be accessible at the place of business).  This would assist larger MT offices and franchises such as Massage Envy that have numerous MTs working at any given time, as being required to put up all of the licenses can be overwhelming (both practically and aesthetically).

In addition, the Attorney General Office's Counsel for the MT Rules Subcommittee, Nicole, noted that the MT rules cannot regulate others, so the reference to sexually oriented businesses in 2(e) will be removed.  If a sexually oriented business utilizes the word "massage" in its advertising to the public in the future, the Attorney General's Office will be able to go after that business regardless of whether the MT rules reference it.  Nicole will also be looking into the provision in 2(e) regarding disclosing compensation in an advertisement, she thought this was overreaching, and will be looking into how other professions regulate advertising.  Nicole will circle back on these matters by the next MT Rules Subcommittee Meeting.

Again, please remember that the 'rules' mentioned above are currently in the discussion phase, and are not the final rules. If you have any comments, questions, or input you are welcome to attend these Rule-Making Subcommittee meetings.

The following was brought forward to the subcommittee as a starting point for discussion
