Root cause analysis (RCA) is an essential process used in healthcare to reduce or eliminate unintended patient harm by implementing effective corrective actions. Effective corrective actions are the result of clearly written root cause contributing factor statements that are identified by thoroughly understanding what happened and why it happened.
This training session will inform and teach participants how to implement a standardized process that identifies root causes and contributing factors as well as effective and sustainable corrective actions to reduce or eliminate future patient harm. This process, known as the RCA

, published by the National Patient Safety Foundation, is endorsed by the Joint Commission and healthcare organizations around the country.
