  • Hosted at rural/under-served geriatrics care sites. Teams interview host site clients; share team plan with staff
  • Mentored by GGEAR www.umaryland.edu/gerontology. Implemented by AHEC West
  • Promotes:  Person-Centered Care, Interprofessional Teamwork, Rural/Underserved Practice, Geriatrics Careers
