New Vehicle for Door-Through-Door Program

" We are extremely grateful to the Fallbrook Rotary club for funding the Door-Through-Door car which will allow us to get clients home from the hospital and for Darlene to conduct pre and post hospitalization visits," says Executive Director Rachel Mason.

Donors made it possible to update The Club's look with fresh paint and a new Smart 60" TV.  North Coast Church gave the gazebo new life by creating an outdoor event area during their Weekend of Service program.

Computer Learning Center Moved 
Crestview Estates Club House at 1120 E Mission Rd is now the location for the Computer Learning Center.
   Stop by between 1-3 Fridays when the Senior Techs are there to help. 
   Classes are also available there for $40 and are two hours long for four weeks.

Newly Launched "Door-through-Door" Program to Break Cycle of Repeated Hospitalizations

"Coming home from the hospital or a stay at a skilled nursing facility should be about healing, not about struggling with lack of food, medicine, equipment or wondering if anyone is coming to help." said Executive Director of the Foundation for Senior Care Rachel Mason. If one of those pieces is missing, the result is often a revolving door back into the hospital.  The recently launched "Door-Through-Door" program has been created to ensure that medical care plans and social support systems are in place before discharge so that clients can safely return home and focus on healing, breaking the cycle of repeated hospitalizations. 

Seniors who have an adult child or friend living nearby who can coordinate resources for them may not need this service . Many Fallbrook seniors, however, do not have anyone to help them. This program fills that void.
Help fund these vital services for seniors & disabled adults. 
Seniors & Disabled Clients Can Turn to "Door-through-Door" Advocate Coordinator for Help to Heal 

"We have always been available to help seniors get necessary medical devices and resources," says Advocate Coordinator Darlene Weber. "This new program expands the advocate's role and creates a focused, dedicated  approach to working with clients and hospitals before, during and after a client is discharged and, if needed, to transport the client after discharge.  

    "The process varies with each individual. Those with insurance and their own funds have different needs from others who are under insured or lack funds. Those who have funds may only need our help to locate convenient suppliers. For clients who lack funds, we work with other non-profit partners and volunteers. Whatever the level of need, the mission of this program is to fill it so that clients know they can return home safely and focus on healing. We coordinate s olutions as though the ones in need were our own family members."

   The Door-through-Door program is such an obvious solution to the communications gap in the health care system, which is why I think we've had such great success from private grant funders to support the program expenses.
 
  Most of the funding for the pilot program, estimated to cost around $64,000, has come from a health service contract from the Fallbrook Regional Health District and other private grant funders.  Our recent "Gangsters & Molls" event finished funding the program for the year plus the increased costs for insurance. 

   To keep this program viable for many years to come, we will be asking for the community's support with ongoing donations. As with all programs available through the foundation, donations and bequests can be earmarked specifically to support the Door-Through-Door program. 

  As our community ages, it is important that we are here to help our neighbors be safe and independent in their own homes for as long as possible."
