Chronic Disease Self-Management Program Leader Training at Keiro

What is Chronic Disease Self-Management Program (CDSMP)?
People with chronic conditions have similar concerns and problems. Understanding that individuals with chronic conditions must deal not only with their disease/s, but also with the impact these have on their lives and emotions Stanford University developed CDSMP. This program supports people living with health conditions such as diabetes and high blood pressure by outlining strategies for managing daily challenges.
 
By helping to improve healthful behaviors participants can improve their health status and enhance their quality of life.
 
Course meets for 6 weeks, 2.5 hours per class, once a week. This program is also available in Japanese.
 
Why become a leader?
As a trained leader you can help others live a genki lifestyle by introducing and practicing strategies to manage their chronic health conditions. A rewarding and powerful program for both leaders and participants where the process (way in which CDSMP) is taught is as important as the subject matter.
 
Japanese-speaking leaders are welcome and encouraged (the training is offered in English).
 
When is the training?
DATES:  July 18, 19, 25 and 26, 2016
TIME:  9:00 a.m. - 4:00 p.m. (for each session)

Noted physician Dr. Thomas T. Yoshikawa, Deputy Chief of Staff for Geriatrics, Extended Care and Special Projects at the Veterans Affairs (VA) Greater Los Angeles Health Care System will be speaking on diabetes, risk factors and prevention.

FREE COURSE
Powerful Tools for Caregivers  is designed to benefit family caregivers

Family members who have been caregivers, or who are current and/or future caregivers for any condition are invited to participate. The course provides participants with tools and strategies to better handle challenges faced by family caregivers.
 
Classes meet six (6) consecutive weeks for 90 minutes per session. Sessions are led by a trained Keiro staff member and volunteers in a small group setting. Space is limited. Registration is required. To register or for more information, please email [email protected]
 
Next session begins Saturday July 16, 2016 (1:00 p.m. - 2:30 p.m.) and meets each Saturday through August 20, 2016.
