Prize Available for Using Data to Improve Population Health 

The Concordium Award for Innovations with Data is designed to spotlight government agencies using innovative tools and displays of data to improve population health. With support from the Robert Wood Johnson Foundation (RWJF), up to three recipients will receive a presentation opportunity, travel stipend, AcademyHealth membership and complimentary registration to Concordium. 


Using Data to Address Social Determinants of Health: Two Approaches

National interest in the use of social determinants of health (SDOH) data to improve patient and population health continues to grow. Call for Creative Research: What Makes America Healthier?  

Our friends at RWJF are looking to fund innovative research studies that explore how policies and efforts in different sectors such as urban planning and criminal justice impact health. This funding opportunity is part of RWJF's Evidence for Action program, committed to developing and refining strategies to build a culture of health. 


Project Spotlights

This month, we're featuring two projects that are integrating multi-sector data into a health information exchange to improve care coordination for high-need patients and reduce emergency department utilization. 

DASH: Center for Health Care Services

Through DASH, the Center for Health Care Services (CHCS) is leading a multi-sector effort to create a connected information system that integrates physical and behavioral health care data into Healthcare Access San Antonio, a regional health information exchange. The system will enable critical sectors, including law enforcement, hospitals, and homeless services providers, to rapidly identify people in crisis and notify CHCS staff through real-time alerts. 


CHP: North Coast Health Improvement and Information Network

This week, the CHP Program is highlighting North Coast Health Improvement and Information Network (NCHIIN), a not-for-profit health information exchange that will work to reduce emergency department utilization by the indigent, high-need super-utilizer population by 15 percent in Humboldt Country, CA. Through its participation in the CHP Program, NCHIIN proposes to develop IT infrastructure and new workflow systems to support clinical data sharing across healthcare and social service sectors to decrease emergency department utilization and expand the data exchange to other programs and social service organizations.

New Reading: Toward the Future of Public Health Systems Research

Last month, AcademyHealth commissioned four discussion papers for the 15th Annual Public Health Systems Research Meeting. The papers draw on the authors' experiences and perspectives on the changing role for public health in the transforming health system. Collectively, they propose a research agenda for improving population health through systems-level change. 
An Environmental Scan of Recent Initiatives Incorporating Social Determinants in Public Health

A new CDC environmental scan identifies, categorizes, and describes an array of multi-sector initiatives and collaborations currently under way nationwide that explicitly include attention to social, economic, and environmental factors to foster community health and well-being.
