Sharing Results Through Pictures 

Since its formation in 1993, Health Foundation of South Florida has been recognized on a local, state and national level for its work to improve health in Broward, Miami-Dade and Monroe Counties. In 2007 the Foundation altered its course and embarked on a new direction in grantmaking. The goal was to affect positive change and impact by focusing on specific community health needs.
 
An extensive planning and strategy development process, which included a convening of community stakeholders, grantees, Foundation staff and Board members, led to the creation of four priority areas:  Healthy Eating Active Communities, Oral Health, Preventive Health Measures and Primary Care. These priority areas were used as a catalyst for the Foundation's five-year responsive grantmaking initiative.
 
A program evaluation of this initiative incorporated lessons learned and outcomes from our grantees, the true drivers of community impact. As a valued community partner, Health Foundation of South Florida shares this information with you. The infographics below represent each of the priority funding area investments made from 2008 to 2012.
 
For its current five-year responsive grant cycle, Health Foundation continues to be responsive to the health needs of residents, as reflected in the addition of our new priority area, Behavioral Health. But there is one thing that will always stay the course: our vision for a South Florida that shines in its ability to improve the quality of life for all residents.
