Summer Mailer 2016/2017

Please check your mailbox for the Summer Mailer - a physical mailer sent out last week with lots of information and forms. Please click HERE to read the cover letter. Below is the 2016-2017 Mailing Checklist, complete with links. You can also access all forms via our website  (password: myschool). If you have any questions, the Yavneh office is open Monday-Friday 8am-3pm.

The following is a list of forms and program sign-ups needed for the beginning of the school year.

Required Forms for all Students  - Due August 15th

Required Forms for New Students - Due August 15th
□ Health Exam Form   (Gan/Alef Families Only)
 
Required Forms for Middle School Families - Due August 15th
          □ PE Shirt Order (order via link by August 5th)
          □ Documentation of Tdap (7th Grade Only)
 
Additional Forms (not required)
