REMINDER: Submit Your VPK Summer 2016 and/or 2016-2017 COE's
Good Afternoon VPK Providers,
 
This is a reminder to submit your VPK Certificates of Eligibility (COEs) for VPK Summer 2016 or VPK Fall 2016-2017 to us promptly.  Once you have at least four (4) COE's for each classroom, they should be submitted using the Provider Portal.

 
How to Submit Your VPK COE's if your 
VPK contract  was completed i n the Provider Portal:
 
  1. Login to your Provider Portal account
  2. Locate the word "Children" at the top of your screen
  3. Once you see the drop-down menu, select "VPK COE's"
  4. Select "VPK Summer 2016 or VPK Agreement 2016-2017" in the Agreement field
  5. Select the appropriate classroom in the Classroom field for the child you are enrolling
  6. Click on the "+ Add New Child" button directly below the Classroom field
  7. Enter all available information and click "Save"
  8. Keep the hard copy COE on file for monitoring/auditing purposes
***If a child needs to be removed from the classroom, in the drop down use "Remove"
***If a child changes classroom before VPK start date, in the drop down use "Edit".  If child changes classroom after the first day of attendance use "Edit" and complete an Informed Parent Consent for Class Transfer form and upload into the Document Libraries VPK Transfer Form folder.


 
How to Submit Your VPK COE's if your
VPK contract was completed manually other than the Provider Portal:
 
  1. Please upload your COE's into the Document Libraries folder named Enrollment_VPK COE's
  2. If a child needs to change classrooms after the first day of child's attendance, you must complete an Informed Parent Consent for Class Transfer form and upload into the VPK Transfer Form folder.
  3. If child(ren) need to be removed from your classrooms, please notate this on a cover letter and upload into the Document Libraries folder named Enrollment_VPK COE's
