Cornerstone VNA recently worked with CGI Communications to create a brief video that showcases the many programs and services that we provide to our patients and the community.

Introducing New Telehealth Units for Patients
Since 1998, Cornerstone VNA has been providing patients with Telehealth, an in-home disease management system to closely monitor chronic illnesses.  Recently, Cornertone VNA introduced a more advanced Telehealth program using a 4G-enabled 10" Samsung tablet.  The Telehealth units monitor vital signs, provide medication reminders, real-time videoconferencing and high-risk alerts. Patients also answer disease specific daily symptom questions through the tablet. With a focus on education, the tablets offer a wide range of videos and information on chronic diseases, which includes congestive heart failure, pulmonary disease and diabetes.  Learn more by reading our brochure or contact our office for additional information.
