MaineHealth ACO Case Management Program Highlighted at the National Association of ACOs
 
Last week, the MaineHealth ACO highlighted a recent innovation in patient risk identification at the National Association of ACOs (NAACOS) Spring Conference in Baltimore, Maryland. NAACOS is a non-profit organization committed to creating "an environment for advocacy and shared learning" among Accountable Care Organizations across the United States. MaineHealth ACO Chief Operating Officer, Jennifer Moore, sits on the NAACOS Board of Directors.

NCQA is in the process of transforming their programs, including Patient Centered Medical Home (PCMH) recognition. Progress and Shared Success from the ACO
 
Complex Care Management Pilot in the Nephrology Setting 

In October, the Value Oversight Committee awarded a grant to Maine Nephrology Associates and Maine Medical Center to pilot a collaborative, complex care management team in the nephrology specialty setting. The project adds RN care management, social work case management, and a nutritionist to the nephrology care team with the goal of helping stage 4 or 5 chronic kidney disease patients better manage their care. The care management team works closely with providers and staff at Maine Nephrology Associates to create a highly functioning team dedicated to the care of these highly complex patients.
