ASIPP 18th Annual Meeting a Tremendous Success
The American Society of Interventional Pain Physicians (ASIPP) 18th annual meeting was held last week in Dallas, TX. The meeting boasted a new record number of attendees.ASIPP's Annual Meeting turned 18 in April with the largest attendance in ASIPP's history. Those who attended the meeting in Dallas are still buzzing about what a great success it was.
The Friday morning kick-off began with a tribute to the late Prithvi Raj.The tribute was given by Dr. Laxmaiah Manchikanti. Dr. Raj touched almost every pain physician's life and millions of patients across the globe.was a world-renowned anesthesiologist and pioneer in the field of regional anesthesia, setting standards for care worldwide. His significant research, many articles, books, lectures and demonstrations on regional anesthesia and pain management proved him to be an international leader in the field.
Following the Tribute, a brief awards ceremony was held. The Rising Stars award was given to: Devi E. Nampiaparampil, MD, and George Chang Chien, DO. Outstanding Service Awards were awarded to: Vidyasagar Pampati, Melinda Martin, Vijay Singh, MD, and C M Schade, MD, PhD, FIPP.
Outstanding State President Awards went to: Harold Dalton, MD, Ray H.D'Amours, MD, Amol Soin, MD, and Graf Hilgenhurst, MD. Outstanding Advocacy Awards were presented to: Donald Jones, MD, Francis Riegler, MD, and Sudhir Diwan, MD. Academic Achievement Awards were given to: Alan Kaye, MD, PhD who is Editor in Chief, Pain Physician; Mark Boswell, MD, PhD, who is Executive Director, ABIPP; and also to Joshua Hirsch, MD, and Kenneth Candido, MD
The League of Presidents awards went to: Dr. Peter Staats who will chair the organization for one year; Ramsin Benyamin, MD, who is a2 time president (2014 & 2009); Hans Hansen, MD (2013); Frank Falco, MD (2012); Standiford Helm, II, MD (2011); Alan Parr, MD (2010); David Schultz, MD (2008); Andrea Trescot, MD (2007); Vijay Singh, MD (2006); David Kloth, MD (2005); and Laxmaiah Manchikanti, MD (1999-2005).
This final and the most prestigious award given by ASIPP, The Lifetime Achievement Award was given to: Laxmaiah Manchikanti, MD. Previous recipients of this award are: Prithvi Raj, MD (2003), Gabor Racz, MD (2004), Samuel J. Hassenbusch, MD, PhD(2005), John D. Loeser, MD (2006), Stanton Hicks, MD (2007), Nikolai Bogduk, MD (2009), and Richard Derby, MD (2011).
New officers for 2016 were announced. Aaron K. Calodney, MD will be President; Francis Riegler, MD is President Elect: David Bryce is First Executive Officer; and Dr. Peter Staats, Md is Immediate Past President.
World-class lecturers have been a principal feature of every ASIPP Annual Meeting and 2016 was no exception. Robert Laszewski provided this year's Manchikanti Distinguished Lecture. "No one has a greater knowledge of the Washington health policy scene than Bob Laszewski --- political, economic, social, and straight merit dimensions. Bob also has uncanny forecasting powers... and he calls the shots exactly as he sees them, unusual for Washington." - John McLaughlin, The McLaughlin Group. He is a national expert on Health Policy. Mr. Laszewski is president of Health Policy and Strategy Associates, Inc. (HPSA ), a policy and marketplace consulting firm specializing in assisting its clients through significant health policy and market changes.
Albert Telfeian, MD, PhD and Laxmaiah Manchikanti, MD presented the Raj-Racz Distinguished Lectures. Albert Telfeian, MD, PhD, is director of pediatric neurosurgery at Hasbro Children's Hospital. He was the recipient of the American Association of Neurologic Surgeons Stereotactic and Functional Neurosurgery Research Award, the American Epilepsy Society Young Investigator Award, and the J. Kiffin Penry Pediatric Epilepsy Award, and was named one of the nation's 15 leaders in neurosurgery in the October 2011 edition of Newsweek. Laxmaiah Manchikanti, MD is the Chairman of the Board and CEO of ASI PP® and SIPMS . He is a highly sought after source by national media and government on IPM issues. He is the author of numerous books and hundreds of articles, as well as a leading advocate for IPM.
We were honored to have the following Keynote speakers; On Friday ASI PP® board member Dr. Devi
Nampiaparampil, 'Doctor Devi', spoke during the Resident/Fellow section. She regularly appears on national TV and other networks. Kenneth Pettine, MD and William D. Murrell, MD spoke on Saturday. Dr. Pettine founded The Spine Institute. Dr. Murrell is an author, innovator, international lecturer, stem cell pioneer, and biologic treatment expert. The meeting keynote speaker for Sunday was Susan Blank, MD, Co-Founder and Chief Medical Officer of the Atlanta Healing Center.
Attendees could choose from three different tracks in the afternoon sessions. On Friday the sessions included: Session A featured Getting Involved for Your Survival: Not Optional, and Business of Pain Medicine: Managing Your Future. Session B featured the Residents/ Fellows Session ; and session C was on Practice Management.On Saturday Session A featured Current Utility and Future of Interventional Techniques, Spinal Cord Stimulation , and Minimally Invasive Techniques. Session B featured the Abstract Session and awards followed by Emerging Concepts and Controversies then Session C was again on Practice Management.
Abstract Awards were given out. 1st place went to presenter Jeff Petersohn for the abstract titled: Innervation of the Anterior Capsule of the Human Knee: Implications for Radiofrequency Ablation;" Second place; went to Ramsin Benyamin, MD for the presentation: "Multicenter Randomized Controlled Pivotal Trial Comparing 10 kHz and Traditional Spinal Cord Stimulation: 24-month Results" and 3rd place went to Dr. Richard Rauck for the presentation :Sustained Effectiveness of Intrathecal Ziconotide Use as the First Agent in Pump in Patients With Severe Chronic Pain."
Sunday was a half-day for attendees. Its theme was Controlled Substance Management features Keynote Speaker Susan Blank on Pathophysiology of Controlled Substance Use, Overuse, Abuse,
Dependency, and Addiction.

The initial impression of ICD-10 implementation was that it went well for both providers and CMS, however, shortly after the October 1, 2015 date when Medicare claims began processing, providers found out otherwise. In some cases, Interventional Pain Management providers continue to struggle with "finding the right code" to report for their patient's condition.
CMS released a publication "Clarifying Questions and Answers Related to the July 6, 2015 CMS/AMA Joint Announcement and Guidance Regarding ICD-10 Flexibilities" http://tinyurl.com/CMS-ICD-10-ClarifyQ-A that provided additional details as to how this ICD-10 family of codes "free pass" was going to work.
In short, CMS stated, "The recent Guidance does not change the coding specificity required by the NCDs and LCDs. Coverage policies that currently require a specific diagnosis under ICD-9 will continue to require a specific diagnosis under ICD-10. It is important to note that these policies will require no greater specificity in ICD-10 than was required in ICD-9, with the exception of laterality, which does not exist in ICD-9. LCDs and NCDs that contain ICD-10 codes for right side, left side or bilateral do not allow for unspecified side."
What this means is that:
- Interventional Pain Management providers are still being held responsible for reporting the correct ICD-10 code or risk Medicare claims denials.
- Many Interventional Pain Management procedures are included in the various Medicare contractor's Local Coverage Policies (LCD) and include very specific lists of ICD-10 codes that meet medical necessity for the diagnostic and therapeutic procedures.
- In some cases, Medicare contractors have separate LCD policies for each type of interventional pain procedure and often multiple pages in length.
Don't be one of the providers that have the misunderstanding if they report a valid ICD-10-CM code in the same family of codes, Medicare will accept that.

Mindfulness Study for Chronic Pain Shows Promising Results
"Everyone in pain wants to get better."
Those words from Dr. Ruben Kalra , a San Francisco Bay Area pain physician whose practice reaches about 25,000 patients every year.
He thinks practicing mindfulness needs to be a bigger part of how physicians treat chronic pain. He and some colleagues studied chronic pain patients and found that doctor-prescribed computerized mindfulness-based interventions (CMBI) earn both a high patient acceptance rate and interest in increasing the use of the therapies.

APS, Pfizer IGLC Partner to Fund Chronic Pain Research
With 100 millions Americans living with chronic pain, the medical community is continuing its efforts to improve the care of patients experiencing this condition.
The American Pain Society (APS) has partnered with Pfizer Independent Grants for Learning and Change (IGLC) to enhance care for individuals with chronic pain by allocating up to $2 million in funding for research grants.

Motor vehicle driving performance of anesthesia residents after overnight work shifts
After six consecutive night shifts, anesthesia residents experienced significantly impaired control of all driving variables including speed, lane position, throttle, and steering in a driving simulator. They also were more likely to be involved in collisions.
In addition, reaction times increased with a significant increase in the number of minor and major lapses in attention.
House staff safety should receive the same careful attention that is given to patient safety, the authors note.

FDA issues three new draft guidances related to compounding of human drugs
 The U.S. Food and Drug Administration (FDA) issued three draft guidance documents related to human drug compounding under the Federal Food, Drug, and Cosmetic Act (FD&C Act), as amended by Title I of the Drug Quality and Security Act (DQSA) in November 2013, that apply to both outsourcing facilities and compounders seeking to operate under section 503A.
The draft guidance documents describe FDA's proposed policies concerning: the prescription requirement in section 503A of the FD&C Act, how the agency intends to apply the prescription requirement in section 503A to compounding in a hospital or health system pharmacy, and the definition of the term "facility" in section 503B of the FD&C Act.

Can Clinicians Be Misled About FDA 'Breakthrough' Drug Designation?
Use of the word "breakthrough" in the US Food and Drug Administration's expedited approval process could mislead doctors about the new drugs' actual benefits, according to a research letter published in the Journal of the American Medical Association.
The U.S. Congress in 2012 gave FDA the power to designate a drug as a "breakthrough therapy" if preliminary clinical evidence suggests an advantage over existing medications. But a survey of 692 doctors revealed that many tended to misinterpret "breakthrough." Doctors often believed the drugs were supported by stronger evidence than the law requires to achieve that designation, lead author Aaron Kesselheim, MD, JD, MPH, a faculty member at Brigham and Women's Hospital in Boston, told HealthDay.

Daily Pain Variations: A Weak Link to Opioid Craving
Prescription opioid use in the United States has reached epidemic proportions, with sales nearly quadrupling over the last decade despite an unchanged prevalence of pain.

The epidemic carries a price - more than 140,000 Americans have died from prescription opioid overdose since 1999, and the estimated cost to society is in excess of $50 billion per year.

In an effort to address the issue, the US Food and Drug Administration has published a comprehensive review and action plan,
