Find, Select, & Visit your In-Plan Primary Care Provider (PCP)

It's important to find, select and make an annual preventive visit appointment with your In-Plan PCP right away
.
Your In-Plan PCP will help you stay at your healthiest by providing annual physical examinations, immunizations, lab tests such as cholesterol screening, and other types of screening tests depending on your age, gender, and overall health status. A PCP can be a family physician, internal medicine physician, pediatrician or a nurse practitioner. Another plus-going to regular annual checkups ensures that your In-Plan PCP becomes familiar with your medical history and general health so they can better treat you in the future.
There are a couple of ways to select an In-Plan PCP.
One way is by calling the Member Services Team at 855-644-1776 Monday through Friday from 8am until 6pm. Another way is by registering and logging onto Minuteman Health Direct, our secure, members-only online portal. To start, visit MinutemanHealth.org and click on the MEMBER LOGIN button on the top menu bar. Make sure you have your ID card handy. You'll need it to register.

Monadnock Community Hospital
|

Minuteman Health is pleased to welcome Monadnock Community Hospital (MCH) to its In-Plan Provider network. Located in Peterborough, New Hampshire, MCH is a 25-bed Critical Access Hospital offering medical, surgical and Intensive Care, Obstetrics, Pediatrics, and Mental Health services. In addition, a wide variety of outpatient services are available, including an extensive Primary Care Network, Cardiac, 24-hour Emergency Care, a fully equipped laboratory and an extensive Radiology department. MCH was named among the "Top 100 Critical Access Hospitals in the US" by ivantagehealth.com in April 2015.
MCH also features the Bond Wellness Center, the region's only medically based fitness and rehabilitation facility, which offers classes, rehabilitation therapy and programs, along with visiting specialty physicians. In 2015, MCH opened a newly expanded Oncology & Infusion Therapy Center, which offers patients advanced cancer care close to home.

Do something different-even a small change like clearing the clutter off your desk can make a big difference when it comes to stress.
Focus on now.
Turn off and tune in. Let technology be a tool that works for you and not the other way around.
Talk to yourself.
Sometimes a good pep talk is all you need to keep stress at bay.
Smile and laugh.
Laughter really is the best medicine! Watch a funny show or video, tell a funny joke, or go to a card shop and read funny cards.
Meditate.
A still mind is a stress-free mind, so take a deep breath, quiet your thoughts, and let the stress melt away.
Get social support.
Call a friend, send an email; share your feelings with someone you trust.

Currently the New Hampshire In-Plan Provider network includes over 11,200 doctors, hospitals, community health centers, and other health care providers. That includes over 8,000 In-Plan Providers in MA and over 3,200 In-Plan Providers in NH.

#MinuteMemberTip 1: Ask for Generics
Generic drugs include FDA-approved drugs dispensed under a name that is not trademarked. These drugs contain the same active ingredients as their brand-name equivalents, and usually cost less.
#MinuteMemberTip 2: Shop Around
Whether at home, on vacation, or away for business, you can fill you prescriptions through Minuteman Health's nationwide network of 50,000 In-Plan Pharmacies. Participating pharmacies include Costco, CVS, Rite Aid, Stop & Shop, Target, Walgreens, and Walmart.
#MinuteMemberTip 3: Use our Mail Order Pharmacy Benefit through OptumRX
We offer two easy ways for members to fill a three-month supply of their maintenance prescriptions (drugs taken daily to manage the symptoms of chronic conditions, such as diabetes and asthma):
- Mail Service: Members may have their maintenance prescriptions delivered right to their homes or offices** through OptumRX's Home Delivery program. Each copay covers up to a 90-day supply of a prescription or refill. (Please note that mail service copays may differ from the standard pharmacy copays.)
- Access 90: Members may fill up to a 90-day supply of maintenance drugs at participating retail pharmacies through our Access 90 program.** Three standard 30-day pharmacy copays apply.
