What You Need to Know From the
Office of Business Affairs
You will be receiving a letter in the mail this week from the Office of Business Affairs directing you to mandatory forms that must be completed, signed and submitted electronically by each student. You may review these forms and instructions on this webpage prior to receiving your letter: www.mountunion.edu/forms-5.

The mandatory forms include:
  • Tuition Payment Agreement, which is an agreement between the student and University regarding payment of tuition, room and board, costs, fees and expenses (if applicable). All students upon enrollment are subject to this agreement.
  • FERPA release that will allow the Office of Business Affairs to speak to only those persons you authorize (typically parents or guardians) about your tuition and payment arrangements.

  • Federal Title IV Authorization form, which will allow the Office to use any federal monies you receive to cover the cost of tuition and board, as well as other miscellaneous fees you might incur, and to hold any credits on your account until a refund request is made.
You will also receive a bookmark from one of our partners, CASHNet. They provide various monthly payment plans that will assist you in budgeting the amount due.

This sounds like a lot, but this is an important next step in becoming a Purple Raider, so be sure to look through the information carefully and submit the necessary documents.

Please contact the Office with any questions at 330-823-6557.
Dual and College Credit Transcript Request
If you intend to bring in credit from a dual credit, PSEO, or College-Credit Plus program, you will need to order an official transcript from the college or university that awarded the credit. Transcripts must be sent to the Office of Admission.
