Using Social Determinants of Health Data to Improve Health Care and Health
In a new report from the Robert Wood Johnson Foundation (RWJF), leaders from across the country, including DASH Co-Directors Clare Tanner and Peter Eckart, shared their perspectives on different approaches to collecting and using social determinants of health data.
Highlights from the CHP In Person Meeting
This week, the CHP Program held its first In-Person Meeting at the National Program Office (NPO) AcademyHealth's headquarters in Washington, DC With over 60 attendees, including the CHP communities, several program advisors, technical assistance providers, and experts from national organizations, the meeting fostered opportunities for networking, peer learning and collaboration.
- Day 1: Wednesday, June 1: The meeting kicked off with opening remarks from AcademyHealth President and CEO Dr. Lisa Simpson and ONC Chief Medical Officer Dr. Mason, who talked about the importance of leveraging data to achieve greater health equity and improve population health. Day one's sessions covered community action plan development, building trust and working toward data sharing agreements, and navigating consent in cross-sector data sharing.
- Day 2: Thursday June 2: Day two began with a presentation from Alison Rein on the current landscape of multi-sector data sharing initiatives, with early results from an ongoing environmental scan conducted by the CHP Program NPO. Other sessions focused on emerging standards and opportunities for aligning social determinant data sharing efforts, as well as insights and advice for communities as they map their data sharing projects to broader reform initiatives. Participants also discussed and prioritized possible learning guide topics.
