A Message from Premier's Chief Executive
Every morning when I walk through the doors at Premier - I am humbled by our dedicated staff who continually go above and beyond, providing the highest quality healthcare services to our community.

We have over 150 committed and compassionate employees that enable Premier to be a vital asset to Pasco County. Each care team member has their own unique story as to why they've chosen a career with Premier. Next time you visit one of our clinics or see a Premier care team member around town - ask them to tell you their story. Odds are, you'll have a heartwarming chat about family, helping others and the importance of improving access to care for our most vulnerable community members.

This commitment to excellence has allowed Premier to grow and offer even more services to Pasco County residents. Here are a few of our new programs/initiatives underway:
  • Patient Care Coordination. This is a value-added program that supports chronically ill patients to achieve their health goals. Licensed nurses connect with patients based on geography and help them manage diabetes, hypertension, asthma or other related health challenges.
  • ER Diversion. Premier recently began placing select care team members in our community partner hospitals. These individuals help patients navigate the healthcare market and also connect patients to medical homes to decrease or prevent avoidable emergency department visits.
  • Dental services in our West Pasco locations. Through a state grant from AHCA, Premier has launched oral health services in West Pasco two times a week which will provide yet another needed service for residents.

Lastly, I'd like to thank our state legislators for their hard work and dedication to the residents of Pasco County. This past Spring, I was fortunate to visit during session in Tallahassee and saw first-hand their support of Premier and their true commitment to our community.

Children's Dental Day at Premier
3 out of every 5 children are affected by tooth decay
"Tooth decay is the most common childhood illness, yet it is entirely preventable," explained Dr. Peralta, Associate Medical and Dental Director at Premier. To help bring awareness to oral health as a priority, Premier recently hosted Children's Dental Day in partnership with the Department of Health-Pasco County at its Dental Center in Dade City.

Oral health care access is an important element to maintain individuals' well-being, both mentally and physically. In alignment with the Healthy People 2020 oral health objectives, Premier recognizes that oral health is vital to overall health and that everyone, regardless of income, should have access to preventive oral health programs.

Peralta further emphasized that tooth decay can be prevented by ensuring one has a dental exam twice a year. Other simple steps include practicing good oral hygiene (brushing & flossing teeth twice a day), and avoiding sugary drinks and candy. Sadly, tooth decay affects school attendance and can often impact other physical health problems.

Improving access to oral health care is one of many important priorities at Premier. It is one of the many reasons Premier's dental services exist in Dade City and most recently started in West Pasco at the Trinity/NPR location.

Lacoochee Family Health Center Celebrates
1-Year Anniversary
!
On April 20th the Lacoochee Family Health Center (a Premier satellite location) celebrated its 1-Year Anniversary. The milestone celebration included brief walking tours of the health center, community resources, hands on activities and healthy snacks.

Open every Wednesday since March 2015, the Lacoochee Family Health Center served nearly 400 patients. Of those 37% were walk-ins (same day appointments). As demand increases, the center hopes to expand its hours of operation.

"Making health care more accessible for area residents was a main goal for the Lacoochee Family Health Center when it opened a year ago," said Joseph Resnick, CEO of Premier. "We're so pleased with the success the Center has seen and hope this milestone will help further establish it as a main source of permanent primary care health services in the local area."
 
Making an Impact: Premier's Patient Care Coordinator Program
Florida Blue Foundation Makes 3-Year Investment
In 2013, the Florida Blue Foundation awarded Premier a grant to launch a new Patient Care Coordination program, 'Connecting People to Care.'

Premier's PCCP achieves its goals by facilitating daily huddles with the care team at the family practice sites and offering several added value services for patients including pre-planning sessions, motivational calls and small group educational sessions. This personalized approach allows patients enhanced access to care and improves overall continuity of care to better manage self-care and maintain a connection to community resources.

Nearly three years since launching the program, it has doubled in size and now includes coordination with the Community Services department at Premier. Patients have been responsive to the program services and physician referrals continue to increase.

When new eligible patients are registered at Premier, the program is presented to them as a free value-added service. Overall, there is better coordination between providers and the Patient Care Coordinators, leading to greater patient satisfaction and medical compliance.


The confidence patients have after engaging with a Patient Care Coordinator has shown promising results. Since the program's inception, Patient Care Coordination services have been provided to almost 2,000 patients resulting in more than 5,000 pre-visit encounters.
