SHF Introduces New Grants Manager
Leslie Bremar, Grants Manager

We are pleased to welcome Leslie to the SHF Team. Leslie began her her role as Grants Manager in January. She will serve as the first point of contact for any questions about the grant application process, including technical questions about how to submit a grant application. 

Leslie was born and raised in Huntington, WV. She earned an undergraduate degree in Political Science from Eastern Kentucky University.   After interning at the Federal Trade Commission and for the Lexington-Fayette Urban County Government, she determined her passions were focused in the nonprofit sector.  Read More

Cynthia Presents at National Rural Health Meeting
Leveraging National Partnerships

On May 23-24, Cynthia attended the Fourth Annual Public-Private Collaborations in Rural Health meeting in Washington, DC.   Since 2012, the gathering has convened over 120 public and private foundations, researchers, and policy makers to discuss federal programs and foundation led-initiatives in rural areas. The purpose of this year's meeting was  to strengthen current activities and leverage regional and national relationships between foundations, researchers, and federal partners. 

Cynthia, along with Kim Tieman of the Benedum Foundation and Alana Knudson of the Walsh Center for Rural Health Analysis, presented on measuring outcomes in public-private collaborations specifically related to the Care Coordination Aligned Funding Initiative of which two West Virginia entities received grant awards. One of the grantees in our service area is the Minnie Hamilton Health System which operates as the lead agency of a ten-agency partnership. The project is utilizing Community Health Workers to assist patients to gain better control of their diabetes in eight WV counties. Funding partners include the Bernard McDonough Foundation, Claude Worthington Benedum Foundation, the Highmark Foundation, and the Sisters Health Foundation.

$7,000 to purchase home blood pressure monitors and to
 support ten scholarships for the Diabetes Prevention Program.

Athens Mental Health; Athens, OH -- $15,095 for capacity building and operational support.

Rural Action; The Plains, OH -- $25,000 to expand their Farm to Institution to School program including the purchase of a new, professional grade vacuum sealer for use at Hocking College.
 
Washington-Morgan Community Action; Marietta, OH -- $5,000 to support their Summer Feeding Programs in New Matamoras and Beverly.
 
West Virginia Healthy Kids and Families Coalition; Charleston, WV -- $10,000 to support two Try This mini-grant projects and a statewide evaluation of the Try This program.

American Friends Service Committee; Milton, WV -- $15,000 to support statewide public policy advocacy including preserving health care gains and child nutrition advocacy.
 
American Heart Association; Charleston, WV --$32,000 to train middle school students in Calhoun, Pleasants, Roane, and Wirt Counties on CPR in Schools and CPR Anytime Training Program.
 
American Red Cross of Northwest West Virginia; Parkersburg, WV -- $14,500 to train high school students in Citizen CPR and to deliver CPR Training Certification to on-site personnel.
 
CASA for Children; Moundsville, WV -- $3,500 to support the Tyler County program in preserving kinship placements and the Fostering Futures initiative for aging out youth.

National Church Residences; Belpre, OH -- $20,000 to implement a transportation service for elderly residents of Putnam Howe Village Apartments to non-emergency appointments. 
 
Ohio University - Heritage College of Osteopathic Medicine; Athens, OH -- $20,000 to purchase patient medication vouchers, diabetes management supplies, and patient lab/diagnostic expenses for their free clinic.
 
TEAM for WV Children, Huntington, WV -- $15,000 to support the newly-formed state ACES Coalition to mobilize collaboration around Adverse Childhood Experiences (ACES) in WV.
 
West Virginia Center on Budget and Policy; Charleston, WV -- $20,000 to support their general operations as they address West Virginia's Medicaid program expansion and sustainability.
 
West Virginians for Affordable Health Care; Charleston, WV -- $30,000 to support their general operations as they educate legislators and local leaders on Medicaid, reach out to local partners to identify uninsured children and adults in Medicaid expansion and/or CHIP, and engage consumers in solving problems related to healthcare financing and health system change.

Basic Needs/Direct Service Grantees

Adams House Ministries; Middlebourne, WV  --  $5,000 for fresh produce vouchers and hygiene products for Tyler County residents.

Catholic Charities West Virginia ; Parkersburg, WV  -- $5,000 to support their case management and Mobile Food Pantry programs.

First Lutheran Church; Parkersburg, WV  -- $1,000 to purchase tools and equipment for their community gardens in Wood County.

Kanawha Valley Fellowship Home; Charleston, WV  --  $4,000 to assist with food expenses for Wood County clients.  

KVC Health Systems; Ripley, WV  -- $4,500 to support new foster families from Jackson, Roane, Wirt, Wood and Calhoun Counties with immediate needs (ie. beds, car seats) for their children.

Lutheran Social Services of Central Ohio; Worthington, OH  -- $5,000 to assist with food expenses for their Mobile Food Pantry in Athens County.

South Parkersburg United Methodist Church; Parkersburg, WV  --  $3,500 to provide healthy food options for their Backpack Food Ministry in three Title 1 schools. 

West Central Regional Drug Court;  Parkersburg, WV  -- $5,000 to support drug court participants with emergency housing/rental assistance and other basic needs.
