It has come to our attention that you have NOT submitted your 2016-2017 School Readiness Contract or missing documents through Provider Portal. 

The Coalition  requested all Contracts and documents to be submitted no later than May 13, 2016 .

To assure that your School Readiness Contract is renewed for the 2016-2017 year on July 1, 2016, we must receive your contract and/or missing documents no later than Friday June 3, 2016. You may submit your School Readiness Contract through Provider Portal.
