Going All In to Bridge the Health Data Gap

How can communities understand all the factors that contribute to their health? The answer is clear: we need a better approach to collecting, sharing and using health data, one that includes sectors beyond health care. Our latest blog outlines why multi-sector data sharing is important, what we know about current efforts, and how the All In network is working to advance the field.


Collaborating with our Communities: The CHP Program Share and Learn Webinar Series 

In March, the CHP Program launched its Share and Learn Webinar Series. Integral to the program's commitment to peer learning and collaboration, the webinar series highlights the work of the CHP Participant Communities and Subject Matter Expert Communities. Each webinar in the series features short presentations by communities sharing goals, examples and ideas for cross-sector collaboration, and data integration efforts.

In April, the CHP Program and DASH joined the Rural Health Community of Practice for a discussion on rural health. Both programs shared their knowledge about efforts to improve the access and quality of health in rural settings. 


Share Your Knowledge: Answer 5 Quick Questions

CHP and DASH are co-presenting at the Practical Playbook National Meeting in May, where we will discuss how local experts are uniquely suited to answer each other's project-related challenges. Therefore, we are asking those with expertise in multi-sector data sharing to respond to a short survey to provide your insights. 

Project Spotlights

This month, we're featuring two multi-sector data sharing projects working to improve health outcomes in rural areas in Minnesota.  

DASH: White Earth Reservation Tribal Council

Through DASH, the White Earth Reservation Tribal Council is expanding implementation of WECARE, a care coordination database system that provides an efficient and effective way to link Native American families to the services they need. The project is improving care coordination by enabling connections between primary care, home health, education, early childhood programs, and human services.


CHP: Minneapolis Heart Institute Foundation

An organization passionate about improving the cardiovascular health of individuals and communities through innovative research and education, the Minneapolis Heart Institute Foundation is one of five CHP Subject Matter Expert (SME) Communities. This SME Community will share lessons learned during the development, implementation, and evaluation of Hearts Beat BackĀ®: The Heart of a New Ulm Project (HONU), a 10-year community-based initiative to reduce heart attacks in New Ulm, Minnesota.

  • Health Datapalooza May 8-11, 2016 in Washington, DC -  This  national conference is focused on liberating health data, and bringing together the companies, startups, academics, government agencies, and individuals with the newest and most innovative and effective uses of health data to improve patient outcomes.
  • NNHPI Annual Conference May 17-19 in New Orleans, LA -  This is the only national meeting that supports and highlights the work of the nation's public health institutes. NNPHI members lead conference programming that explores fresh concepts and strategies for supporting healthy communities at the local, state, and federal levels.
  • Practical Playbook National Meeting May 22-24 in Bethesda, MD -  By bringing together key stakeholders from across sectors, this meeting will help to catalyze a national movement, accelerate collaborations by fostering skill development, and facilitate the exchange of ideas to drive population health improvement.
  • National Health and Human Services Summit May 22-25 in Arlington, VA -  This national summit will provide a robust platform to fully engage all sectors of health and human services. The workshops and sessions will encompass a diverse set of topics ranging from policy to research to state and local initiatives.
  • Concordium 2016: Sept. 21-22 in Arlington, VA - This AcademyHealth conference brings together individuals and organizations working with big health data to integrate evidence, practice, and policy in the delivery system setting.
