CEO Update

Population Health/Accountable Care Organizations (ACOs) - by Geneseo Republic
For years, hospitals have received reimbursement for services provided ... but now, hospitals could receive financial incentives if patients don't need hospital care.
 

"Our world really will be turned upside down," said Hammond-Henry Hospital chief executive officer Brad Solberg.

As part of the federal government's Affordable Care Act, health care organizations such as Hammond-Henry are being encouraged to work together to demonstrate a better coordination of care.

 

The entities are known as Accountable Care Organizations (ACOs).  According to the federal government, "The goal of coordinated care is to ensure that patients, especially the chronically ill, get the right care at the right time, while avoiding unnecessary duplication of services and preventing medical errors."

 

Education is key to an ACO's success, said Solberg. "There will be incentives to make sure communities are healthy," he said. "With an ACO, you need to become more proactive."

Free2Play

Hammond Henry Hospital is proud to announce a Free2Play (F2P) Family Fitness Morning for K-8 students and their families. "The Free2Play program empowers every child to be an athlete. It cultivates each child's natural abilities (Perspire) and personal capacity to contribute to the wellbeing of others (Inspire)." Our goal for the morning is to help kids develop a greater appreciation for exercise and use their gifts and talents to help encourage their friends, families, and communities. F2P is a trusted world-class movement literacy program developed by the Gray Institute.

Patient Portal

We are pleased to announce that Hammond-Henry's new Patient Portal called My Chart is now available. This online tool will give you the flexibility to access your health information and other resources.

My Chart will be at your convenience and is available on the Internet, which means that it can be accessed from virtually anywhere.

Foundation Update

'Celebrity Dinner' Fundraiser  

The 11th Annual Hammond-Henry Hospital Foundation Celebrity Dinner fundraiser was held on Monday, April 21 at the Cellar. The event had 25 local celebrity waiters & waitresses and 160 additional participants. The goal was to raise $30,000 to be used for completing the north patio area in the Long-Term Care Living Center. Total funds raised have surpassed the goal. Darcy Hepner, Foundation manager, attributes the success of this event to the support from the area community members.  

This event has raised over $300,000 since its inception. These funds have been used for various hospital equipment and projects to benefit our local communities. For more information on the Foundation go to www.hammondhenry.com

Volunteer Update

National Volunteer Week - April 6 - 12    
We recognized our volunteers at an annual appreciation luncheon on April 8 at First Lutheran Church. There were 108 attendees. We salute our volunteers at Hammond-Henry Hospital and recognize the valuable contributions volunteers make in our community!  Volunteers are a long and proud tradition and a vital part of the service provided to patients and visitors at Hammond-Henry Hospital. In 2013, there were 240 adults and 58 teens who volunteered on a regular basis in a variety of roles. At HHH you can find volunteers on the shuttle in the parking lot, pushing a wheel chair or giving directions, setting up displays in the HHH Gift Shop, playing cards with residents in  Long Term Care, delivering mail or flowers, assisting with a mailing, greeting patients, playing the piano, or sitting with patients. Together volunteers and Auxiliary members logged a total of 18,032 hours last year as they not only volunteered in the hospital, but they also raised funds for scholarships for local students in health care careers. In 2013, Auxiliary volunteers also donated $50,679 toward equipment and scholarships from funds raised in the HHH Gift Shop and from various fundraisers. 
  

And what do volunteers get out of the experience?  Volunteers have a chance to learn new skills, meet new people, develop   new interests, and give something back to  the community. Most of all, they get the satisfaction that comes from helping others. Thank you volunteers!
