Using a self-developed approach that combines elements of Geisinger’s Proven Health� Navigator, Johns Hopkins Guided Care Nursing and the Wagner Chronic Care Model, Taconic Professional Resources is assisting physician practices in the New York Hudson Valley to improve population health and care for their sickest patients through the use of embedded RN case managers.

Taconic supports, manages, trains and evaluates embedded case managers for physician practices. It started with an 18-month pilot program and moved into an expanded model that serves the CMS Innovation Center’s Comprehensive Primary Care (CPC) initiative—as well as physician practices that function in medical home and ACO structures.

During Improving Population Health with Embedded Case Managers in Independent Physician Practices, an October 9th webinar at 1:30 pm, Annette Watson, senior vice president of community transformation for Taconic, will share how embedded case managers have been successfully deployed in an open, multi-payor community.

Upon completion of the program, participants will be able to:

  • Explain how to evaluate the needs of a practice for an embedded case manager;
  • Describe the various roles of embedded case managers in physician practices, from supporting CPC to meeting ACO and PCMH requirements;
  • Address funding strategies for an embedded case manager program; Discuss patient referral sources and workflow strategies to maximize the embedded case manager function; and
  • Share effective ways to use electronic medical records, patient registries, payor data and other tools within a practice to support the embedded case manager.

Taylor will also share the impact of these initiatives on emergency room utilization, quality scores and gaps in care.
