What We Learned: HIN's fourth annual industry survey on healthcare case management, administered in May 2013, captured data on case management responsibilities, diagnosis most responsive to case management, challenges of embedded case management, and much more. According to the 118 healthcare companies who responded, the top care sites where case managers are embedded, besides primary care, are:

• Hospital: 52.3 percent
  
  
• Community: 20.5 percent
  
  
• Clinic: 13.6 percent
  
  
• Other: 11.4 percent
  
  
• Home care: 11.4 percent
  
  
• Long-term care: 9.1 percent
