Controlled Substance Management Course and Competency Exam

Update on PQRS Implementation

Among multiple regulations, we are also faced with documentation for the physician quality reporting system. Now the bonuses have ended and penalties are being imposed on non-participation starting last year. If you did not complete them for 2014, you will see a 2% reduction in Medicare fee-for-service amounts for services furnished during 2016. The same is true for 2015; if you did not complete documentation for 2015, you will face a penalty in 2017 and 2016 will result in a penalty in 2018. After 2018, the PQRS payment adjustment will be transitioned to a merit-based incentive payment system, or MIPS, as required by the Medicare Access and CHIP Reauthorization Act (MACRA) of 2015.
 
We have attempted to make it palatable for interventional pain physicians to meet the criteria. Recently, we published a manuscript on Physician Quality Reporting System (PQRS) for Interventional Pain Management Practices: Challenges and Opportunities (click here for manuscript).

 
Essentially, PQRS is one of the easiest to implement among all the regulations. Enclosed, please find the recommended measures for IPM providers. There are 9 measures included and 3 optional measures. The included measures are utilized by the undersigned

A Seamless Navigation to ICD-10-CM for Interventional Pain Physicians: Is a Rude Awakening Avoidable?
 
Since October 1, 2015, the International Classification of Diseases, 10th Revision, Clinical Modification (ICD-10-CM) was integrated into U.S. medical practices. This monumental transition seemingly occurred rather unceremoniously, despite significant opposition and reservations having been expressed by the provider community. In prior publications, we have described various survival strategies for interventional pain physicians.
 
The regulators and beneficiaries of system - CMS, consultants, and health information technology industry are congratulating themselves for a job well done. Nonetheless, this transition comes at an immeasurable financial and psychological drain on providers. However, a rude awakening may be making its way with expiration of initial concessions from government and private payers.
 
This manuscript provides a template for interventional pain management professionals with multiple steps for seamless navigation, including descriptions of the most commonly used codes, navigation through ICD-10-CM manual, steps for correct coding, and finally, detailed coding descriptions for various interventional techniques.

SAN FRANCISCO (Reuters) -- With a backlash brewing over the price of medicines in the U.S., drugmakers are pushing back with a new message: Most people don't pay retail.

Top executives from Eli Lilly, Merck, and Biogen said in interviews with  Reuters   this week that the media focus on retail, or "list prices," for branded medications is misplaced.

Twenty-four year-old Aaron Nowiski died alone, on his bed next to two bags of heroin.

The Army veteran, who served two tours in Iraq, had secretly been using the powerfully addictive drug, even fooling his family into the understanding that he had quit, before it claimed his life in 2011.

Committee Recommends FDA Approval of Opioid Addiction Implant
 
The Psychopharmacologic Drugs Advisory Committee has recommended that the Food and Drug Administration approve an implant for treating opioid addiction.
The committee voted 12 to 5 in favor of approving probuphine, the first long-acting, subdermal buprenorphine implant to treat addiction to opioids. Buprenorphine is currently only available either as a dissolvable film that is placed under the tongue, or a pill, which allows some addicts to sell their supplies to get money for drugs.
 
Because probuphine works like a contraceptive implant, with four small implanted rods that provide a steady amount of medication for up to six months, its manufacturer Braeburn Pharmaceuticals says the implant eliminates that risk.

The Power of Emotion on Chronic Pain
 
BL is a 76-year-old male patient who suffers from lower back pain that radiates into the lower extremities. He has had lumbar surgery; however, it was not successful. When BL was first accepted as a new patient, he was on high doses of hydromorphone. This was reported to be the only opioid agent that adequately controlled his pain. 

Over the past year, BL lost his wife to Alzheimer's disease, which was very traumatic for him. Shortly after his wife died, he became acquainted with another woman close to his age and subsequently began dating. Shortly after the relationship started, his new friend was diagnosed with terminal cancer. She, too, died shortly after the diagnosis was made.

No Antibiotics for Colds, Sore Throats
 
Two doctor groups are reminding physicians to use antibiotics sparingly this cold and flu season.

The American College of Physicians and the CDC urged doctors not to dole out antibiotics for colds, sore throats, bronchitis, and sinus infections that aren't complicated, according to guidance published in the Annals of Internal Medicine.
