IMPACT Planning & Evaluation Supports Integrated Healthcare Initiative in Milwaukee

Research has shown that people with mental illness and substance abuse disorders experience higher morbidity and mortality than the general population, due in part to increased risks of chronic health conditions such as diabetes, obesity, smoking, and hypertension. The Substance Abuse and Mental Health Services Administration (SAMHSA) created the Primary and Behavioral Health Care Integration  (PBHCI) grant program to address these health disparities, enhance the quality of care, and reduce/control the cost of care.
 
IMPACT's Planning and Evaluation team is currently partnering with the Milwaukee Center for Independence  
(MCFI) to evaluate an integrated care initiative aimed at improving the health of individuals served through MCFI's Whole Health Clinical Group . In the fall of 2015, MCFI was awarded a four-year PBHCI grant from SAMHSA to integrate primary care and peer-led health promotion services into their existing behavioral health programs for Milwaukee County residents with severe mental illness.
 
Working in partnership with the program and MCFI's internal evaluator, IMPACT's evaluation will assist the project in meeting SAMHSA's data collection and reporting requirements; describe the implementation of the integrated primary/behavioral healthcare model; document the project's progress towards its goals and objectives; and provide routine feedback to inform program improvements.
