Medication:
The Medication Permission Form is required for all medications, including over-the-counter, prescription, holistic and herbal. The Health Office stocks acetaminophen, ibuprofen, Calamine lotion, aloe vera lotion, Benadryl for allergic reactions, saline solution, antibiotic ointment, and Cepacol sore throat lozenges. Parents may provide other medications to be administered regularly or as needed, provided the medication is in its original container with the student's name, accompanied with the appropriate order from a physician.
The Medication Permission form must be completed by the parent/guardian AND your child's health care provider to receive the stock medicine. By state law, Registered Nurses are not allowed to dispense any medication without an order from a Health Care Provider for the new school year.
Physicals:
Although the school recommends yearly physicals for all students, a physical is required
by New York State if: by September 8, 2015 or by the first day of sports practice it has been over 12 months since the last physical and:
- your child plays sports for the school
- your child is a new entrant
- your child is entering Kindergarten, or grade two, four, seven or ten.
Immunizations:
Children entering grade six are required to have a Tdap vaccine. This vaccine is given between the ages of 10-11 years old. A second MMR is due by kindergarten.
