Submitting SR Contracts is now Easier
with the Provider Portal
Good Afternoon Providers:

The ELC recently informed you that all SR Providers are required to use the Provider Portal to submit your 2016-2017 SR Provider Contract.   The contracts are now available on Portal for you to complete.

The new Provider Portal will allow you to execute your 2016-2017 SR contract and submit supporting documents electronically.  Renewal contracts are due to the Coalition no later than May 2, 2016 for the 2016-17 year.

When you login to the Provider Portal just select the "2016-2017 SR Contracts".  The contract is in an editable PDF format that is easy to use. Your contract will never be submitted incomplete. If you leave out information in any section of the contract, a message box will pop up letting you know what still needs to be completed.

Once complete, you can sign your contract electronically and submit it to us. You can also upload supporting documents to us such as Certificates of Insurance, Workers Comp certificates, Delegation of Authority forms, etc. These documents will be stored in your personal folders within the Provider Portal to access when you need them.

If you are one of the providers not yet using the Portal, setting up an account is easy. You need a valid email address, your child care license number or main phone number, and zip code of the location.

  • If you have any questions or need assistance with the provider Portal, please contact our Help Desk at (727) 400-4474 or [email protected]

The Help Desk can also assist any provider who needs training on completing attendance, uploading documents, and the other features of Portal.

If you have any questions about completing your SR Contract, please contact your assigned SR Contract Specialist.

Thank you for the work you do every day to care for and educate young children in our community. We're confident the Provider Portal will make your job a little easier.
