Change to Request for Justification of Excessive Absences Form
In effort to streamline services to families and providers, the Coalition will no longer require the use of "mommy notes" when children have illnesses requiring them to stay at home. 
 
This change will take effect with the February 2016 attendance rosters that are due by the March deadline.
 
Please note that you will still be required to provide a doctor's note or hospital admissions record when the child or parent has been hospitalized.  
 
The Coalition has also revised the Request for Justification of Excessive Absences form to reflect this change in process.  Please note that this change ONLY affects the SR absence procedure and NOT VPK.   To download a copy of the revised form, click here or you can find a copy of the form at www.elcpinellas.net/sr-downloads, under Reimbursement.
