Can this happen again?
With the current disaster in Flint Michigan, most US citizens are questioning the safety of their own drinking water. Let us not forget that two years ago, West Virginia faced a similar disaster when a chemical spill in Charleston occurred one mile from the state's largest water treatment center; therefore causing one of the largest chemical contamination incidents in our nation's history.
The disasters in Flint and Charleston are not located in the realm of technical errors; rather, the governance and accountability by government institutions and private sector companies
in some of America's most vulnerable places.
How can this be prevented? It all comes down to HR policies and management.
