C2C Innovative Solutions, Inc. Transitions to Nonprofit Status 
 
C2C Innovative Solutions Inc., a subsidiary of TMF Health Quality Institute, has transitioned to a nonprofit status and added "Innovative" to its corporate name.
 
C2C is a Qualified Independent Contractor for the Medicare program. It specializes in providing health care sector support and administrative services in the handling of Medicare second-level appeals and payment resolution disputes between health care providers and insurance companies. 
CMS Awards TMF
Two Special Innovation Projects
 
The Centers for Medicare & Medicaid Services (CMS) awarded TMF Health Quality Institute two Special Innovation Projects (SIPs) that focus on health care quality improvement, align with the goals of the CMS Quality Strategy , and emphasize the power of partnerships.
 
The two-year contracts were among 16 SIPs awarded to 10 different regional Quality Innovation Network-Quality Improvement Organizations (QIN-QIOs) such as TMF. The 16 SIPs have a combined value of just under $10 million.
 
Medication Adherence Contract
TMF was awarded a project in which we will use education campaigns (patient/physician/pharmacist) to improve medication adherence for diabetes, hypertension and cholesterol medications in six municipalities of Puerto Rico. TMF also will provide technical assistance to increase e-prescribing in a select group of recruited physicians. The planned outcome is improvement in Medicare Part D claims for medication adherence (diabetes medications, hypertension and cholesterol) and physician use of e-prescribing.
 
Improve Early Detection and Management of Sepsis
TMF also was awarded a project in which we will work with 10 hospitals in two Texas communities to help promote an early detection screening tool to identify Systemic Inflammatory Response Syndrome and to promote timely implementation of evidence-based sepsis bundles.
 
The planned outcome of this project is improvement in fee-for-service (FFS) inpatient claims for severe sepsis or septic shock per 1,000 FFS Medicare beneficiaries and per inpatient stay. Also, as a process measure, the project looks for improvement in the percent of recruited hospitals implementing a screening tool, percent of recruited hospitals implementing three-hour sepsis bundles, and percent of recruited hospitals implementing six-hour sepsis bundles.

TMF continues to build its reservoir of patient success stories related to work under our primary contract with the Centers for Medicare & Medicaid Services (CMS).
 
Although improving population health has always been the primary objective of CMS' Quality Improvement Organizations (QIOs), such as TMF, CMS has increased its focus on presenting the faces of success.
 
Tom Manley, TMF president and CEO, regularly shares patient success stories with CMS, as do the other heads of QIO organizations.

Health Care Coalition Teamwork Gets Reluctant Patient to Rehabilitation
TMF Honored for Hiring Veterans
and People with Disabilities

This year marks the 25th anniversary of the Americans with Disabilities Act and TMF is proud to have been honored by Enable America.
 
The organization presented TMF with a plaque stating that we are being honored as an "Employment Partner for supporting the employment of veterans and people with disabilities." In addition, an accompanying letter states: "We also recognize those organizations who are making us a strategic partner in their diversity and inclusion outreach by joining our corporate membership program and providing needed funding to sustain our operations."
 
TMF will continue the good work of providing opportunities for veterans and persons with disabilities.

Enable America is a nonprofit organization founded in 2002 that is dedicated to increasing employment among the 56 million Americans with disabilities.
TMF-Supported Practice Talks Coordinated Care in DHHS Blog

TMF is the national learning network contractor for the Comprehensive Primary Care (CPC) initiative of the Center for Medicare & Medicaid Innovation. TMF develops and delivers curriculum to guide primary practices in broadening their services as they seek to lower health care costs, improve patient health and improve patients' and families' experience with health care services.
 
Recently, the U.S. Department of Health and Human Services blog featured a CPC practice from rural Arkansas that is delivering high-quality primary care partly through an expansion of its coordinated care services.
