Homebound Coordinator/Assessment Aide

6 hours/day Monday-Friday- Northridge, CA.  Area

 

Summary:
The Homebound Meal Driver delivers the appropriate meals to participants in the Meals to the Homebound program in an efficient, productive, and organized manner. The Meals to the Homebound Program delivers hot, nutritious meals to the Company's senior citizen and disabled clientele who are incapable of being transported to any of the Company's centers or satellite locations.

Essential Duties and Responsibilities:
Assures meals are provided to the Company's homebound senior citizen and disabled clientele in accordance with Federal and State nutritional and safety regulations, and Company policies and procedures.
Participates in the unloading and sorting of food delivered to the Centers to assure that all items are accounted for.
Assures meals and snacks are appetizing and appealing, and prepared according to the menus.
Maintains and submits personal documentation to assure current licensure and insurance.
Assures that food is properly loaded into vehicle, utilizing the necessary storage equipment for hot and cold items.
Personally takes temperature of food items at beginning of shift and throughout delivery period.
Handles food in accordance with food safety regulations.
Assures all infection and contamination control procedures are adhered to in the preparation and distribution of food and equipment.
Informs Homebound Coordinator of any changes in status or eligibility of clients utilizing the Meals to the Homebound Program.
Informs Homebound Coordinator of a client's potentially dangerous living conditions that may have been witnessed in the course of carrying out duties.
Collects client donations and submits them to Homebound Coordinator for deposit.
Maintains vehicle and equipment, to ensure cleanliness and functionality.
Records and keeps track of meals served and undelivered meals.
Communicate and engage with clients to create a comfortable and open dialogue about their needs and concerns.
May perform other duties as assigned or requested.

Non-Essential Duties:
Participates in company outreach and marketing, educating the senior community about the availability and benefits of the services provided by the company.
Assists Homebound Coordinator with quarterly ride-along evaluations of Driver performance
Assists CNS representatives with monthly ride-along evaluations of Meals to the Homebound program.
Submits time and mileage reports.
Complies with expectations as demonstrated in the employee handbook.
Demonstrates ability to diffuse and respond to customer concerns to avoid escalation of the problem.

Job Qualifications:
Experience / Knowledge: 
High school degree or equivalent required.
Previous non-profit organization experience preferred.
Minimum two years working experience in a food production setting preferred
Basic written skills required.
Excellent oral communication skills required.
Positive interpersonal skills required.
Fluency in verbal and written English required.
Fluency in verbal and written Spanish preferred.
Ability to utilize kitchen and meal delivery equipment.
Familiarity with the neighborhoods served by the program.

Veterinarians for Afghanistan and Iraq

The Veterinarian is a member of a field veterinarian and veterinarian technician team responsible for the overall canine health of canines supporting the Department of State Explosive Detection Canine teams.  Performance locations will be OCONUS in both Iraq and Afghanistan.

 

All services shall be provided in accordance with established standards, principles and ethics of the profession, applicable professional specialty organizations, and the high-quality standard for which CHSi is recognized.

 

Required Experience
Minimum Position Requirements
Shall possess or be able to obtain a minimum MRPT or SECRET level clearance.
Shall have a degree from a 4-year program at an accredited college of veterinary medicine
Shall be licensed as a Doctor of Veterinary Medicine (D.V.M or V.M.D.)
Shall have a minimum experience of two (2) years as a veterinarian in a private practice or the military
Shall have knowledge of the chemical composition, structure, and properties of substances and of the chemical processes and transformations that they undergo (this includes uses of chemicals and their interactions, danger signs, production techniques, and disposal methods)
Knowledge of plant and animal organisms, their tissues, cells, functions, interdependencies, and interactions with each other and the environment.
Experience in maintaining canine health records and canine international health certificates
Shall have experience with a microchip program that scans, reads and tracks canines
Shall be willing to live and work in conflict/combat areas or similar hazardous environments

Position Duties
Examine animals to detect nature of diseases or injuries.
Treat sick or injured animals by prescribing medication, setting bones,    dressing wounds, or performing surgery.
Inoculate animals against various diseases such as rabies and distemper.
Collect body tissue, feces, blood, urine, or other body fluids for examination and analysis.
Operate diagnostic equipment such as radiographic and ultrasound   equipment, and interpret the resulting images.
Advise handlers regarding sanitary, feeding, and general care necessary to promote health of animals.
Educate handlers/trainers about diseases that can be spread from animals to humans.
Train and supervise workers who handle and care for animals.
Euthanize animals at the direction of the COR.
Establish and conduct quarantine and testing procedures that prevent the spread of diseases to other animals or to humans, and that comply with applicable government regulations.
Conduct postmortem studies and analyses to determine the cause of animals' deaths.
Perform administrative duties such as scheduling appointments and maintaining records.
Inspect and test animals to detect the presence of communicable diseases.
Research diseases to which animals could be susceptible.
Inspect animal housing to determine cleanliness and adequacy.
Determine the effects of drug therapies, antibiotics, or new surgical techniques by testing them on animals
Work effectively independently, however be capable to give medical advice telephonically to a staff of Veterinarian Technicians that are operating at remote locations.

OTHER REQUIREMENTS:
Must have a valid, current U.S. Driver's License
Must have a current U.S. Passport.
Must sign a CHS Foreign Service Agreement (FSA) and is bound to the terms and conditions of the FSA.
Shall be proficient in the ability to speak, write and communicate in English.
Work hours are based on a 72 hr. work week; typically six 12 hour days but other variations may apply.
Requires a favorably adjudicated Moderate Risk Public Trust (MRPT) investigation or Secret clearance conducted by the Department of State, Bureau of Diplomatic Security.
Shall Attend a Foreign Affairs Counter Terrorism training course prior to deployment to Iraq for work on the MSSI-Iraq contract. Training may include: Combat First Aid, Weapons
Familiarization, Defensive Tactics/Driving, IED recognition, Surveillance Detection and Hostage
Survival and Personnel Recovery.
Shall demonstrate proficiency in the use of basic word processing programs and the CHS Medical
Management System software.
Ability to communicate clearly and effectively, both verbally and in writing.

PHYSICAL REQUIREMENTS:
Must be confirmed by a dental and medical examination to be "medically and dentally fit" in  accordance with CHS requirements.
Work is normally performed in a typical interior/office work environment.
The climate is desert-like, hot, and prone to dust storms.

Job Description:
The Department of State (DOS) and the Department of Defense (DOD) will complete a transition from DOD-led U.S. presence in Afghanistan to a DOS-led U.S. presence under the  Afghanistan Life Support Services (AliSS) contract.  The ALiSS establishes a clinically safe and professionally managed healthcare environment and provides primary health care as well as resuscitative surgical care in accordance with U.S. or equivalent medical standards on a 24-hour basis, 365 days of the year to Chief of Mission (COM) personnel in Afghanistan.
This position is notional to contract award.
SCOPE:
The Biomedical Equipment Technician reports to and is supervised by the AliSS Program Manager or a designated Project Management Office (PMO) official and coordinates their work with the site medical director or other primary medical equipment users (Radiologist, Physicians, Surgeons, Nurses, Nurse Practitioners, Physician's Assistants, Medical Technologists) on the work site
The Biomedical Equipment Technician will be assigned to repair or maintain equipment in one of three settings: (1) Large medical/trauma hospitals, (2) Small medical/trauma hospital and assists in examination and treatment of patients under the direction of a physician, (3) Non-surgical field Health units.
In these settings, Medical Maintenance Specialists install, repair and maintain medical equipment to ensure it is clean, safe and functioning properly. This includes Biomedical equipment that requires use of computers with specialized software, hand tools and other repair shop equipment to diagnose and fix problems with a wide variety of healthcare technology. Work on X-ray and ultrasound machines, respirators and heart defibrillators, electric wheelchairs and hospital beds, or medical laboratory equipment is also required. Biomedical Equipment Technician will assist with Medical equipment set up and disinfect laboratory and healthcare equipment so it is safe and ready to use.
All services shall be provided in accordance with established standards, principles and ethics of the profession, applicable professional specialty organizations, and the high-quality standard for which CHS is recognized.

Required Skills
QUALIFICATIONS:

Education
Must have completed a Biomedical Equipment Technology or Engineering training program leading to a diploma, certificate or an associate's degree.
U.S. Military Biomedical Equipment Technician technical school training certification (4A2X1, 68A, or HM-8478) will also fulfill this requirement.
Experience
Have three years of experience
Job Specific Qualifications and Requirements
Must have additional training and experience repairing CT scanners and similar sophisticated equipment.
Must have and maintain current certification in Cardio Pulmonary Resuscitation (CPR).

Additional duties:
Biomedical Equipment Technician may be required to complete their work near patients and must avoid disturbing them.
Biomedical Equipment Technician will document trouble and other reports, the work performed, and any system or configurations changes.
Attends and participates in a variety of meetings and task force groups to improve quality and outcomes, communicate issues, obtain approvals, resolve problems and maintain specific level of knowledge pertaining to new developments, new task efforts, and new policy requirements.
Shall understand and practice the precepts of the American Hospital Associations Bill of Rights for patients.
Other duties as assigned.
All Health Care Providers (HCPs) will document care delivered and follow up care required.
Attends and participates in a variety of meetings and task force groups to improve quality and outcomes, communicate issues, obtain approvals, resolve problems and maintain specific level of knowledge pertaining to new developments, new task efforts, and new policy requirements.

 

OTHER REQUIREMENTS:
Shall be licensed/certificated in the United States
Participate in training, (including HIPAA training) that may be required by DOS or CHS prior to deployment to Iraq.
Shall complete a one week Foreign Affairs Counter Terrorism training course prior to deployment to work on the contract. Training will include: Combat First Aid, Weapons Familiarization, Defensive Tactics/Driving, IED recognition, Surveillance Detection and Hostage Survival and Personnel Recovery
All Bio Med Techs will require a favorably adjudicated Moderate Risk Public Trust (MRPT) investigation conducted by the Department of State, Bureau of Diplomatic Security;
Medical Maintenance Specialists will sign a CHS MSSI Foreign Service Agreement (FSA) and is bound to the terms and conditions of the FSA.
Must have a valid, current U.S. Driver's License
Must have a current U.S. Passport.
Shall be proficient in the ability to speak, write and communicate in English.
Shall demonstrate proficiency in the use of basic word processing programs and the CHS Medical Management System software.
The normal workweek is established by the DoS Chief of Mission.

 

PHYSICAL REQUIREMENTS:
Must be confirmed by a  medical examination to be "medically and dentally fit" in accordance with      DOS requirements for service in Iraq
Work is normally performed in a typical interior/office work environment;  travel in Iraq is required by surface or air;
The climate is desert-like, hot, and prone to dust storms.

Diagnose patients with common acute conditions, illnesses or minor trauma
Request x-rays and laboratory tests as deemed necessary
Plan: Formulate a health care plan for patients presenting for treatment, emphasizing self-care responsibility through the participation of the patient, family, physician and other health care professionals
Intervention: Treat patients with common acute conditions, illnesses or minor trauma within accepted protocols and/or in collaboration with a physician
Identify resources and coordinate referrals for patients requiring further evaluation and services
Evaluation: Modify the health care plan as warranted, implement and participate in follow-up and return to work programs
Adhere to departmental and clinical safety guidelines
Prepare and submit appropriate documentation as specified in applicable regulations
Liaison and report to higher facility as required for patient's continuum of care
On a regular and continuous basis, exercises administrative judgment and assumes responsibility for decisions, consequences and results having an impact on people, costs and/or quality of service within the functional area
Meet all Joint Commission standards

Joint Base Balad Medical Support establishes a clinically safe and professionally managed healthcare environment and provides primary health care in accordance with U.S. or equivalent medical standards on a 24-hour basis, 365 days of the year as part of the Base Life Support services.

 

SCOPE:
This position description is applicable to all Western Trained Medical Doctors (WTMD) who may be selected. The Western Trained Medical Doctor reports to the Joint Base Balad Medical Support Program Manager and is supervised by the designated site medical director and may be a:
*Emergency Medicine Specialist (defined by current board certification) with a broad interest and experience in providing primary care across the range of office-based specialties including, but not limited to, GYN, dermatology, ENT, pulmonary, cardiovascular, ophthalmology, orthopedics, psychology and gastroenterology.

*Family Medicine Specialist (defined by current board certified or equivalent board certification) with interests as defined above and with an interest, additional training and practice experience to include the broad range of gynecologic, medical, surgical, orthopedic, psychiatric and trauma emergency.

*Internal Medicine Specialist with interests as defined above and with an interest, additional training and practice experience to include the broad range of gynecologic, medical, surgical, orthopedic, psychiatric and trauma emergencies.

The WTMD will treat patients in these settings to ensure proper injury care and disease diagnosis, treatment and recovery, request necessary tests and follow-up visits and refer patients to specialists as necessary. Some settings will require the WTMD to coordinate with local hospitals to determine local capabilities, coordinate admission, monitor and arrange final disposition. In addition, some settings will require stabilization of patient and coordination of evacuation of patient to medevac center or CONUS. In some locations, the WTMD will have a leadership role and serve as both a Physician and Site Medical Director or Chief Medical Officer in support of a Program Manager.

The WTMD provide a U.S. standard of care for our patient care/treatment model.

 

QUALIFICATIONS:

Education
Graduate of accredited US Medical School or equivalent international school.

Experience
Have a minimum of 5 years post residency medical practice experience in US.
Job Specific Qualifications and Requirements
Hold and maintain a current MD or DO unrestricted U.S. (State) license
Must be AME (Aviation Medical Examiner) certified.
Residency, or similar training, successfully completed in a US Hospital or equivalent international hospital in any one of three specialties: Family Practice, General Internal Medicine, or Emergency Medicine dependent on primary employment purpose
Have and maintain current certification in Advanced Cardiac Life Support (ACLS), Advanced Trauma Life Support (ATLS), and Cardio Pulmonary Resuscitation (CPR).
All medical personnel must have worked 6 of the last 12 months performing direct patient care in their career field to be considered.

Additional duties:

Maintain HIPPA certification.
