Howard County Health Assessment Survey Has Begun
The Howard County Health Assessment is underway. This second phase of the ongoing comprehensive health assessment is designed to learn more about the health of Howard County residents. The survey, which was initially conducted in 2012, is a collaborative project of the Howard County Health Department, The Horizon Foundation, Howard County General Hospital and Columbia Association. The survey is comprised of several questions that cover a number of health priorities. The topics were selected to gather data on health behaviors, access to health care, demographical health disparities and progress on local implementation of the Affordable Care Act. You may receive a call to either your cellular or landline telephone. Telephones with Caller ID capability will display the number 410-313-8016 and the text will read "Health Survey." All personal information collected will be strictly confidential and will not be used to identify survey respondents in any way. Please "Answer the Call" and "Make a Difference" in the health of our County. Thank you!
